外科围手术期预防性应用抗菌药物的调查分析

目的：调查我院围手术期抗菌药物预防性应用情况并进行合理性评价。方法：对我院2007年12月1日～31日出院的193例手术患者围手术期抗菌药物预防性使用情况进行回顾性调查分析。结果：围手术期抗菌药物的使用率为99.5%,其中合理56例（29.0%）、基本合理43例（22.3%）、不合理94例（48.7%）。不合理的主要原因为术后用药时间过长、药物选择不当、用药剂量过大、用药时机不当。结论：围手术期抗菌药物的预防性使用仍有不合理现象存在。因此,应加强贯彻落实《抗菌药物临床应用指导原则》,促进预防用药的规范化。

Survey of Prophylactic Utilization of Antimicrobials in Surgical Patients During Perioperative Period

OBJECTIVE： To investigate the prophylactic utilization of antimicrobials in perioperative period in our hospital and to evaluate the rationality of drug use. METHODS ： The perioperative prophylactic utilization of antimicrobials in 193 surgery patients who were discharged from our hospital from the Dec. 1 to Dec. 31 in 2007 were investigated and analyzed retrospectively. RESULTS： The percentage of patients who received antimicrobials during perioperative period was 99.5 %, of which, rational use was noted in 29.0% （56 cases）, basically rational use in 22.3 % （43 cases） and irrational use in 48.7 % （94 cases）. The irrational use manifested as prolonged postoperative use of antimicrobials, incorrect choice of drugs, overdose and inappropriate timing of administration. CONCLUSION： The phenomena of irrational prophylactic use of antimierobials in perioperative period can still be noted. Therefore, it is necessary to implement ＂Guidelines on Clinical Use of Antimicrobials＂ and to standardize prophylactic administration of antibacterials.
