血浆溶血磷脂酸在进展性脑梗死诊断中的应用及阿斯匹林对其含量的影响

摘    要:目的探讨溶血磷脂酸(LPA)在进展性脑梗死发病过程中的变化及临床意义。方法动态监测35例进展性脑梗死患者血浆溶血磷脂酸浓度,分析其与梗死部位、大小、病程、病情的相关性,并分别与稳定性脑梗死、健康体检者对照。结果进展性脑梗死患者血浆溶血磷脂酸急性期升高,发病3d达高峰,以后逐渐下降,7d下降明显,21d接近正常对照,与稳定性脑梗死组和对照组相比,有非常显著意义(P<0.01)。梗死体积大者,则血浆溶血磷脂酸浓度高(P<0.01)。重型组血浆溶血磷脂酸浓度高于轻型组(P<0.01)。皮质组与皮质下组相比无显著意义(P>0.05)。结论血浆溶血磷脂酸浓度与梗死大小、病程、病情严重程度相关,进展性脑梗死患者血浆溶血磷脂酸高于稳定性脑梗死组和对照组。溶血磷脂酸可作为脑梗死的预警因子,对防治缺血性脑血管病,具有重要的临床应用价值。而口服阿司匹林可有效降低缺血性脑血管病患者的血浆LPA水平。

Abstract:Objective To investigate the changes and clinical meaning of lysoophosphatidic acid(LPA) in the course of progressive infarction. Methods The plasm LPA concentration of 32 patients with healthy control respectively,and its relation to the infarction location,the size and the course was studied. Results The LPA level of patients with progressive infarction increased in acute stage,and reached the peak at 3 day after onset,and decreased remarkably after 7 day and nearly normalized at 21days.The results were significantly different from those of stable and heath group(P0.01).The LPA concentration was higher in patients with bigger size infarction.( P0.01)and more serious symptoms (P0.05).Conclusion The LPA level was related to the infarction size,course and the severity,and higher in the progressive group. LPA could be a prognostic factor for cerebral infarction and clinically valuable for treating ischemic cerebrovascular disease. Aspirin could decreased the LPA level of patients with ischemic cerebrovascular effectively.
