摘    要:目的探讨甲亢性心脏病(HHD)患者的心脏形态和血流动力学改变.方法应用多普勒超声测量73例HHD和75例健康对照组各心腔大小、瓣口血流速度和瓣膜返流,评价左室收缩功能,并利用三尖瓣返流速度计算肺动脉收缩压.结果 HHD患者82.7%有不同程度房室腔扩大,左室收缩功能大多正常或高于正常;88%HHD存在瓣口返流,其中二尖瓣、三尖瓣中-重度返流; 14例HHD患者有瓣膜增厚,27例可见二尖瓣脱垂;肺动脉增宽和肺动脉高压发生率分别为62.6% 和73.3%.结论 HHD可引起不同房室腔扩大,左房扩大最常见,右室扩大多于左室扩大;HHD常合并瓣膜反流,部分患者瓣膜增厚;肺动脉增宽和肺动脉高压是HHD的普遍现象.

Abstract:Objective To investigate morphological and hemodynamic changes of the heart in the patients with hyperthyroid heart disease (HHD). Methods Seventy-three patients with HHD and 75 healthy subjects (NS) were examined by Doppler echocardiography. The morphological data, valvular function, left ventricular systolic function and hemodynamic parameters of HHD and NS were studied. Results Among 73 patients with HHD, there were 82.7% with cardiac enlargement, but the majority of their left ventricular systolic function were normal or higher. The valvular regurgitations were found in 88% cases with moderate to severe mitral regurgitation or tricuspid regurgitation. Mild valve thickening and mitral valve prolapse were detected in 14 and 27 cases respectively. Expanded pulmonary arteries and pulmonary hypertensions were detected in 47 and 55 cases respectively. Conclusion Most of HHD lead to cardiac enlargement. Left atrial enlargement can be found mostly, and right ventricular enlargement is more often than left ventricular enlargement. The valvular regurgitations, expanded pulmonary artery and pulmonary hypertension are common manifestations of HHD.
