中药化腐与黄连纱条治疗褥疮局部组织中碱性成纤维细胞生长因子的研究

目的 观察褥疮患者局部组织的碱性成纤维细胞生长因子(bFGF)的变化,探索其形成机制,并观察中药对褥疮患者局部组织bFGF的影响,以阐明祛腐生肌中药治疗褥疮的作用机制,丰富和拓展中医外用药的内容.方法 按照随机数字表的方法将我院2007年11月至2009年4月门诊和住院的褥疮患者分成2组,治疗组(57例)和对照组(52例).治疗组采用提脓祛腐生肌中药,对照组采用西医常规换药方法用相应的抗生素静脉滴注及外用冲洗.2组均取分泌物做细菌培养加药敏试验,以1个月为1疗程,分别观察1、2个疗程前、后的溃疡面愈合情况,溃疡面肉芽组织细胞中bFGF的变化.结果 治疗组总有效率93.0%(53/57),对照组76.9% (40/52),治疗组明显高于对照组(P<0.05).治疗组在治疗1、2个疗程后bFGF(0.8076±0.0389)、(0.7521±0.0816)ng/L]的表达均高于对照组(0.5677±0.0873)、(0.6108±0.0261)ng/L](P均<0.01).结论 褥疮局部应用中药化腐与黄连纱条可以明显提高患者局部组织中bFGF表达,加速创面愈合,提高疗效.

Study of the basic fibroblast growth factor in decubitus tissue treating with Qufu Shengji ointment

Objective To observe the change of basic fibroblast growth factor (bFGF) in the tissue of decubitus region and explore its developing mechanism.To assess the influence of traditional Chinese medicine on bFGF in the tissue of decubitus region,and to elucidate the mechanism of the stagnation-removing and muscle-producing ointment (Qufu Shengji ointment) in treating decubitus.Methods One hundred and seven patients with decubitus treated in our hospital from November,2007 to April,2009 were randomly divided into two groups:57 patients in the treatment group and 52 patients in the control group.The treatment group was treated with Qufu Shengji ointment,and the control group was treated with regular antibiotic injection combined with rinse.Secretion drawn from patients in both groups was undertaken bacteria culture and antibiotics sensitive experiment.The healing of the decubitus was observed at the end of the first and second month,and bFGF in the granulation tissue was measured.Results The overall efficacy was 93.0%(53/57) in the treatment group,which was significantly higher than that in the control group 76.9%(40/52).At the end of the first and second month,the expression of bFGF was (0.8076±0.0389) ng/L and (0.7521±0.0816)ng/L,respectively,which were significantly hisher than that of the control group((0.5677±0.0873)ng/L and (0.6108±0.0261)ng/L,respectively) (P<0.01).Conclusions Application of stagnation-removing tradition Chinese medicine and Chinese goldthread will remarkably improve the expression of bFGF in the decubitus region,and accelerate the healing of decubitus.
