肾和输尿管上段结石采用输尿管软镜联合钬激光碎石术治疗的临床观察

目的分析探讨肾和输尿管上段结石采用输尿管软镜联合钬激光碎石术治疗的临床疗效。方法选取我院近年来收治的80例肾和输尿管上段结石患者作为研究对象,按照患者治疗方法将其分为研究组与对照组,每组40例,研究组患者行输尿管软镜联合钬激光碎石术治疗,对照组患者经皮肾穿刺取石术治疗,比较两组患者治疗效果。结果研究组患者治疗成功率为92.5%,术后一个月结石清除率为95.0%,对照组患者治疗成功率为90.0%,术后结石清除率为97.5%,两组患者手术成功率与结石清除率比较无显著差异（P〉0.05）;研究组患者平均手术时长为（45.0±4.0）min,平均住院时长为（3.0±0.5）d,术后并发症发生率为7.5%,对照组患者平均手术时长为（81.0±6.5）min,平均住院时长为（10.5±0.6）d,术后并发症发生率为15.0%,研究组患者平均手术时长、术后平均住院时长及并发症发生率等指标明显优于对照组（P〈0.05）,组间比较差异显著,有统计学意义。结论输尿管软镜联合钬激光碎石术治疗肾和输尿管上段结石具有较高的碎石成功率与结石清除率,有效性与安全性均有保证,可推广应用。

Clinical observation of renal and upper ureteral calculi with ureteroscopy and Holium laser lithotripsy treatment

Objective To investigate the renal and upper ureteral calculi using flexible ureteroscope holmium laser lithotripsy clinical efficacy of treatment. Methods In recent years, the hospital treated 80 cases of patients with renal and ureteral stones on the segment as the research object, according to the method of treatment patients were divided into study group and the control group,40 patients in each study group underwent flexible ureteroscope holmium laser lithotripsy, a control group of patients percutaneous lithotripsy treatment, treatment groups were compared. Results The two groups of patients without surgical success rate and stone clearance rate significantly different（P〉0.05）; the study group patients undergoing long, long hospital stay and postoperative complication rates and other indicators than the control group（P〈0.05）, significant difference between the groups was statistically significant. Conclusion Flexible ureteroscope holmium laser lithotripsy treatment of renal and upper ureteral calculi with higher success rates with rubble stone clearance rate, efficacy and safety are guaranteed, can be widely applied.
