尿液核基质蛋白22测定诊断尿路移行细胞癌

背景与目的:目前尿路移行细胞肿瘤缺乏有效的无创监测方法。近年来发现的核基质蛋白22(nuclearmatrixprotein22,NMP22)能有效地诊断膀胱移行细胞癌。本研究探讨NMP22作为一个新的尿液中的肿瘤标志物在尿路移行细胞癌筛选诊断中的意义及其影响因素。方法:采用酶联免疫吸附试验NMP22试剂盒,对包括29例尿路移行细胞癌在内的87例泌尿系疾病患者尿液进行的NMP22测定。有尿路感染的患者排除在外。结果:以>10u/ml为阳性判断标准,NMP22对尿路移行细胞癌的敏感性和特异性分别为86.2%和94.3%(以泌尿系良性疾病作对照),远优于尿细胞学检查(86.2%vs42.3%,P<0.001)。引起假阳性的主要因素有尿路感染、泌尿系其他恶性肿瘤、肠道膀胱和肾结石等。结论:尿NMP22是一个较好的肿瘤标志物,可以替代尿液细胞学检查成为尿路移行细胞癌诊断的良好指标。

Significance of urinary nuclear matrix protein 22 in diagnosis of transitional cell carcinoma of urinary tract

Background &Objective:There is no effective non-invasive monitoring method for transitional c ell carcinoma of urinary tract(TCCU).It was recently reported that nucle ar matrix protein 22(NMP22)could be used to diagnose TCCU.This study was design ed to evaluate the significance of NM P22in TCCU as a new urinary tumor mark er and its influence factors.Methods:The authors detected 87patients with urinary system disease(including 29cases with TCCU)with ELISA-NMP agent box.The patien ts with urinary infection were exclu ded.Results:The positive value was set at more than 10u /ml.The sensitivity and specificity of NMP22on TCCU were 86.2%a nd 94.3%,respectively(benign diseases of urinary system used as control),with significant advantages over v oided-urine cytology(86.2%vs 42.3%,P<0.001).The main factors that caused false-positive results were urinary infec tion,the other malignant tumors of u rinary system,bowel interposition,and nephrolith.Conclusion:Urinary NMP22is a good tumor marker a nd could be used an alternation of voided-urine cytology for monito ring and diagnosis of TCCU.
