乳腺良性病变治疗方法的选择

目的探讨应用美容切口治疗乳腺良性病变的效果。方法对458例乳腺良性病变患者,分别采用乳房表面放射状切口(131例)、经乳晕周围弧形切口(157例)、环形切口(83例)及M ammo-tom e微创旋切系统(87例)治疗。结果458例共543处良性病变,均完整切除。病理结果:乳腺纤维腺瘤226例,乳腺腺病瘤样增生172例,乳腺囊肿48例,乳管内乳头状瘤9例,乳腺导管扩张3例。术后门诊随访0.5~1.0年,均无复发及并发症,乳房无下垂及凹陷。放射状切口瘢痕宽0.5~1.5 cm,其他切口宽度均<0.5 cm,M ammotom e术后瘢痕宽度<0.1 cm。结论经乳晕周围弧、环形切口及Mammotome微创旋切系统切除乳腺良性病变,效果可靠,术后瘢痕纤细,对乳房外形无破坏,是较为理想的方法。

Strategy for Benign Breast Tumor

Objective To investigate the strategy for benign breast tumor.Methods 458 cases of benign breast tumor underwent resection via varied incision,including radial incision in 131 cases,para areola incision in 157 cases,ring-shaped incision in 83 cases and Mammotome in 87 cases.The appearance of breast and scar were assessed 6 to 12 months postoperatively.Results Pathological exam showed fibroadenoma of breast in 226 cases,adenosis of breast in 172 cases,lacteal cyst in 48 cases,intraductal papillary epithelioma in 9 cases,ductal ectasia in 3 cases.No recurrence observed within 0.5 to 1.0 year follow-up.No prolapse or complication was occurred.The width of scar for radial incision was 0.5 to 1.5cm, whereas the rest incision was less than 0.5 cm.The scar of Mammotome was less than 0.1 cm.Conclusion Mammotone and para areola incision are the best cosmetic way for benign breast tumor.
