四神丸对腹泻型肠易激综合征模型大鼠及离体结肠的实验研究

摘    要:目的探讨四神丸(SSW)对腹泻型肠易激综合征(IBS-D)模型大鼠腹泻和内脏敏感性及离体分为对照组、模型组、SSW高剂量组(7.2 g·kg-1灌胃)、SSW中剂量组(3.6 g·kg-1灌胃),SSW低剂量组(1.8 g·kg-1灌胃),匹维溴铵组(15 mg/kg灌胃),灌胃每天1次,共4周。用腹壁逃避反射(AWR)评估结肠高敏感性变化,用粪便粒数和腹泻指数评估大鼠腹泻状况,同时观察不同浓度SSW混悬液对大鼠离体结肠自发收缩、乙酰胆碱(Ach)和高钾引发的痉挛性收缩的作用。结果与对照组比较,模型组AWR评分、痛觉压力阈值、维溴铵组痛觉压力阈值均明显升高(P<0.01);SSW低、中、高剂量组和匹维溴铵组粪便粒数及腹泻指数均明显下降(P<0.05或0.01);SSW(0.75,1.5,3,6 g·L-1)对Ach及高钾促发的张力性收缩均有浓度依赖的舒张作用(P<0.01)。结论四神丸能够改善IBS-D模型的排便功能和痛觉过敏,其作用机制可能是通过钙通道竞争拮抗作用抑制结肠运动亢进和肠痉挛。

Abstract:Objective To study the effect of Sishen Pill (SSW) on diarrhea and visceral sensitivity of diarrhea-predominant irritable bowel syndrome (IBS-D) model rats and isolated colon motility.Methods The rats randomly divided into control group, model group, SSW high-dose Group (7.2 g·kg-1) and SSW medium-dose group (3.6 g·kg-1) , SSW low-dose group (1.8 g·kg-1) and in pinaverium bromide group(15 mg·kg-1) were given intragastric administration once a day for 4 weeks. The Colon hypersensitivity was evaluated by the Abdominal Wall Withdrawal Reflex (AWR), and the diarrhea was evaluated by the fecal particle number and diarrhea index, at the same time, the effects of SSW suspension of different concentrations on the spontaneous contraction, acetylcholine (Ach) and high potassium -induced spasmodic contraction of isolated rat colon were observed.Results there were significant differences in AWR score, Pain Threshold, fecal particle number and diarrhea index between the Model Group and the control group (P < 0.05 or 0.01). Compared with the model group, the pain threshold of SSW groups and pinaverium bromide group were significantly higher (P < 0.05 or 0.01), and the fecal granule number and diarrhea index of SSW group and pinaverium bromide group were significantly lower (P < 0.05 or 0.01). SSW (0.75, 1.5, 3, 6 g·L-1) had a concentration-dependent relaxing effect on Ach and high potassium-induced tonic contraction (P < 0.01). Conclusion SSW can improve defecation function and nociceptive hypersensitivity in IBS-D model, and its mechanism of action may be the inhibition of colonic hypermotility and intestinal spasm through competitive antagonism of calcium channels.
