脊髓内海绵状血管瘤的MR影像诊断

目的 探讨脊髓内海绵状血管瘤的MR影像表现特点.资料与方法 经临床证实的11例脊髓内海绵状血管瘤,其中7例经病理证实,男4例,女7例,年龄9～62岁.主要临床表现有反复发作性背部疼痛、肢体疼痛、感觉、运动障碍.MR扫描行矢状位T_1WI、T_2WI,横轴位T_2WI,冠状位T_1WI,增强扫描6例.结果 9例病灶为单发,2例为2个病灶,病灶位于颈段脊髓内8个、胸段脊髓内5个.11个病灶呈类圆形、卵圆形,2个病灶呈梭形.病灶均呈稍长T_1、长T_2信号,其中1个卵圆形病灶、2个梭形病灶内见点条状稍短T_1、短T_2信号分隔,周边均绕以形态不一的短T_2信号环,1个病灶周围有稍长T_1、长T_2信号水肿.6例增强检查,2例病灶未见明显强化,4例病灶显示轻中度不均匀强化.结论 脊髓内海绵状血管瘤MR表现形式具有一定特征性,对提高该病诊断准确率具有价值.

The MRI Diagnosis of Cavernous Hemangioma in Spinal Cord

Objective To investigate the MRI features of cavrnous hemangioma in spinal cord.Materials and Methods Eleven patients(4 males,7 female,9-62 years old) of cavrnous hemangioma in spinal cord confirmed by clinical and pathology data were retrospectively analysed.Clinical manifestations included recurrent attacks of back pain,extremity pain and sensorimotor disorder.All the patients performed plain MRI and six patients performed contrasted MRI scan using routine T_1WI,T_2WI on sagital,transverse,and coronal view.Results Solitary lesion was found in 9 cases and two lesions,were found in 2 cases respectively,Eight lesions located at cervical level spinal cord,and five lesion located at thoracic level spinal cord.Ten lesions had round or ovate shape and two lesions had fusiform shape.All lesions were slightly long T_1 and long T_2 signal intensity.Puncto-striatus form with slightly short T_1 short T_2 signal intensity were observed in one ovate lesion and two fusiform shape lesions.One lesion were surrounded by oedema with long T_1 and long T_2 signal intensity.Among six enhancement cases,2 lesions were not obviously enhanced And 4 lesions were slight to moderately enhanced.Conclusion Cavrnous hemangioma in spinal cord has some characteristic MRI features.MRI has important value in diagnosis of cavrnous hemangioma in spinal cord.
