Clinical comparison of three ways of the hysteromyomectomy

Objective To compare the clinical value of transabdominal myomectomy(TAM),transvaginal myomeetomy(TVM)and laparoscopic myomeetomy(LM).Methods A retrospective analysis 98 patients of our hospital from May 2007 to May 2010 with uterine myomectomy,29 cases throught abdominal myomectomy(open group),vaginal myomectomy 34(vaginal group) and laparoscopic myomectomy 35 cases(laparoscopic group).Results All three operation groups were successful,the vaginal operation time(79±29.4) min was significantly shorter(P<0.05) than the laparotomy group(95±35) min and the laparoscopic group(118±39.2) min;vaginal type group,open group,the laparoscopic group blood loss was(95±26) ml,(143±45) ml,(163±39) ml,the vaginal group than in open and laparotomy group was significantly reduced(P<0.05);The vaginal group(20.5±4.3) h has the shortest ventilation time in three groups,the difference between three groups was statistically significance(P<0.05);About the longest diameter of the myomas,the group of open surgery(8.6±2.6) cm which was significantly longer than the that of the other two groups(P<0.05);The length of hospital stay of the open surgery group(10.2±2.7) d was significantly longer than that of the other two groups(P<0.05);surgical complication rate in the three groups were no significant difference(P>0.05).There were no significant difference in relapse rate among the three groups after 2-36m follow-up,it was about 20%.Conclusion Open group for large diameter of fibroids removed.Both vaginal and laparoscopy groups have advantages of micro-invasion,rapid recovery and short hospital stay time,the three groups could well remove the tumor.
