慢性乙型肝炎患者CD4/CCR5、CD4/CD30细胞亚群失衡研究

目的研究辅助性T细胞亚群（rIh）在慢性乙型肝炎（CHB）发病机制中的作用，寻找治疗CHB的新方法。方法用流式细胞仪对20例正常人（NC组）及20例CHB患者外周血单核细胞（PBMC）经植物血凝素（PHA）培养前后的Th1、Th2水平进行了检测，Th1、Th2细胞表面特异性标志分别为CD4／CCR5、CD4／CD30。结果培养前Th1在NC组与CHB组无明显差别（P〉0．05），CHB组Th2明显高于NC组，同时CHB组及NC组Th1均明显高于Th2（P〈0．05）；培养后，NC组的Th1较培养前明显减少（P〈0．05），而NC组的Th2、CHB组的Th1及Th2较培养前均明显增加（P〈0.05），而以CHB组的Th2增加更明显；培养后，CHB组Th1、Th2均高于NC组（P〈0．05），而CHB组及NC组的Th1与Th2之间则无明显差别（P〉0．05）；培养前CHB组明显低于NC组（P〈0．05），而培养后CHB组高于NC组（P〈0．05），2组培养后Th1／Th2均明显低于培养前（P〈0．05）。结论CHB患者体内Th细胞亚群的应答模式与正常人群存在一定的差异；Th1向Th2细胞的漂移与乙型肝炎的慢性化及反复肝损伤有一定的关系。

Study on the unbalance of CD4/CCR5 and CD4/CD30 cells in patients with chronic hepatitis B

Objective To investigate the roles of T helper(Th) cell subtypes in the pathogenesis of chronic hepatitis B(CHB) to help find new therapies for CHB.?Methods The expression levels of CD4/CCR5(Th1) and CD4/CD30(Th2) of PMBC were assayed by flow cytometry in 20 CHB patients and 20 normal controls(NC) before and after PHA cultivation.CD4/CCR5 and CD4/CD30 are the specific cell surface markers for Th1 and Th2 respectively.?Results Before cultivation,there was no discrepancy in Th1 between NC and CHB(P>0.05);Th2 was higher in CHB than in NC;and Th1 was higher than Th2 in both CHB and NC(P<0.05).After cultivation,Th1 was decreased,but Th2 was increased in NC;while Th1 and Th2 were both increased greatly in CHB,especially Th2(P<0.05).Th1 and Th2 in CHB were both higher than those in NC.There was no discrepancy between Th1 and Th2 in NC or CHB.There was no discrepancy in the value of Th1/ Th2 ratio between NC and CHB before cultivation,but after cultivation,Th1/Th2 in CHB was higher than that in NC,and became lower in both groups than before cultivation.?Conclusion Th subtype responsive model of CHB is different from that of NC;a shift of Th1 cells toward Th2 cells might play some roles in the pathogenesis of chronic viral hepatitis and liver damage.
