唐山地区乙肝患者HBsAg滴度、HBV DNA水平与乙肝基因突变位点的关系研究

摘    要:目的:研究唐山地区乙型肝炎(乙肝)患者乙肝表面抗原(HBsAg)滴度、乙肝病毒基因(HBV DNA)水平与乙肝基因突变位点的关系。方法:选择2018年1月—2020年12月唐山地区150例乙肝患者作为研究对象,测定HBsAg滴度和血清HBV DNA水平,通过基因测序分析突变情况,并据此分为突变组与未突变组,比较不同基因突变位点患者的HBsAg滴度、HBV DNA水平,使用线性回归分析唐山地区乙肝患者HBsAg滴度、HBV DNA水平与乙肝基因突变位点的关系。结果:本研究150例乙肝患者中,有62例患者发生了基因突变,基因突变发生率为41.33%(62/150)。其中,rtM204I/V位点基因突变占比最高,为27.42%(17/62),rtL180M次之,为22.58%(14/62);rtL180M+rtM204I/V、rtA181T+rtN236T基因突变患者HBsAg滴度、HBV DNA水平高于rtM204I/V、rtL180M、rtN236T基因突变患者,差异有统计学意义(P<0.05);经Pearson相关性分析结果显示,基因突变乙肝患者HBsAg滴度、HBV DNA水平...

Abstract:Objective:To study the relationship between hepatitis B virus surface antigen(HBsAg)titer,hepatitis B virus gene(HBV DNA)level and hepatitis B gene mutation site in patients with hepatitis B virus(hepatitis B)in Tangshan area.Methods:One hundred and fifty patients with hepatitis B in Tangshan area from January 2018 to December 2020 were selected as the research subjects.HBsAg titer and serum HBV DNA level were measured,the mutation was analyzed by gene sequencing and divided them into a mutation group and a non mutation group,HBsAg titer and HBV DNA level of patients with different gene mutation sites were compared,Linear regression analysis was used to analyze the relationship between hepatitis B virus HBsAg titer,HBV DNA level and hepatitis B gene mutation site in patients with hepatitis B in Tangshan area.Results:One hundred and fifty cases of patients with hepatitis B,62 patients had gene mutation,the incidence of gene mutation was 41.33%(62/150).Among them,the proportion of rtM204 I/V mutation was the highest,accounting for 27.42%(17/62),followed by rtL180 M,accounting for 22.58%(14/62);the HBsAg titer and HBV DNA level of patients with rtL180 M+rtM204 I/V,rtA181 T+rtN236 T gene mutation were higher than those of patients with rtM204 I/V,rtL180 M,rtN236 T gene mutation,with a statistical significant difference(P<0.05);through Pearson correlation analysis,the results showed that there was a positive correlation between HBsAg titer and HBV DNA level in hepatitis B patients with gene mutation(P<0.05).The HBsAg titer and HBV DNA level of hepatitis B patients with gene mutation were all higher than those of hepatitis B patients without gene mutation,with a statistically significant difference(P<0.05).By linear regression analysis,the results showed that gene mutation might be the influencing factors of HBsAg titer and HBV DNA level(P<0.05).Conclusion:The incidence of gene mutation in hepatitis B patients in Tangshan area is higher,and the gene mutation will affect the HBsAg titer and HBV DNA level of hepatitis B patients.And there are differences in HBsAg titer and HBV DNA level among patients with different gene mutation sites,the two are positively correlated.
