腹腔镜下广泛子宫切除加盆腔淋巴结清扫术治疗早期宫颈癌的疗效观察

目的验证腹腔镜下广泛子宫切除加盆腔淋巴结清扫术治疗早期宫颈癌疗效佳。方法选择我院确诊为早期宫颈癌患者156例,按照患者及家属意愿分为腹腔镜组及开腹组,分别施以腹腔镜下或开腹广泛子宫切除并盆腔淋巴结清扫,术中观察两组患者出血量、脏器损伤等,术后观察两组排气情况、术后感染发生情况等,比较两组治疗效果。结果治疗后腹腔镜组在术中出血量方面较开腹组明显偏少,经统计学分析t=8.63,P<0.05,其他方面均未见明显差异;术后腹腔镜组消化系恢复时间及下床活动时间均较开腹组短,经统计学分析比较t值分别为6、7.71,P<0.05,有差异,余下两项经比较未见明显差异,P>0.05。结论在腹腔镜下行广泛子宫切除并盆腔淋巴结清扫疗效较常规开腹治疗疗效更肯定,与手术相关各方面均较传统治疗佳,受到医患双方满意,适宜临床广泛推广应用。

Clinical Effect Observation of Extensive Hysterectomy under Laparoscope combined Pelvic Lymphadenectomy for Treatment of Early Stage Cervical Cancer

Objective To verify the good clinical effect of extensive hysterectomy under laparoscope combined pelvic lymphadenectomy for treatment of early stage cervical cancer. Method 156 patients with verified early stage cervical cancer were chosen and divided into laparoscope group and laparotomy group according to wills of patients and their families. The 2 groups were respectively given extensive hysterectomy under laparoscope combined pelvic lymphadenectomy and extensive hysterectomy through laparotomy combined pelvic lymphadenectomy. Bleeding amount and visceral organ injuries of the 2 groups were observed during operations and exhaust conditions and postoperative infection conditions of the 2 groups were observed after operations. Curative effects of the 2 groups were compared. Result Total excellent and good rate of laparoscope group was 92.1% obviously higher than laparotomy group 77.8% （P〈0.05）. Conclusion Extensive hysterectomy under laparoscope combined pelvic lymphadenectomy has a better clinical effect than extensive hysterectomy through laparotomy combined pelvic lymphadenectomy. It is better than traditional treatment on every aspect relevant to operation and it receives satisfactions of both doctors and patients, so it is suitable to be popularized in application.
