子宫内膜不均的宫腔镜及病理分析

目的研究超声提示子宫内膜不均伴有临床症状的患者是否患有子宫内膜病变,探讨阴道超声联合宫腔镜检查在子宫内膜病变诊断中的作用。方法对月经异常,不规则阴道流血患者进行阴道超声检查,并对超声检查显示子宫内膜不均的患者177例进行宫腔镜检查,行定点活检和诊断性刮宫,将病理结果与超声和宫腔镜诊断进行比较。结果病理显示异常子宫内膜85例（48%）,异常子宫内膜厚度与正常内膜厚度差异无统计学意义,异常与正常子宫内膜血流差异有显著意义,宫腔镜诊断内膜病变的敏感性为95.3%（81/85）,特异性39.1%（36/92）,阳性预测值59.1%（81/137）,阴性预测值90%（36/40）。结论宫腔镜对子宫内膜不均病因诊断特异性低,子宫内膜异常血流对内膜病变诊断有重要意义。
