盐酸戊乙奎醚治疗重度有机磷毒物中毒临床研究

摘    要:目的探讨盐酸戊乙奎醚治疗重度有机磷毒物中毒临床疗效。方法将68例重度有机磷毒物中毒患者随机分为对照组与观察组,两组患者均给予洗胃、解磷定等常规治疗。对照组32例患者接受阿托品治疗,观察组36例患者接受盐酸戊乙奎醚治疗。比较两组患者人院期间死亡率、治愈率、起效时间、机械通气时间、胆碱酯酶恢复时间、症状缓解时间、住院时间。结果观察组治愈率显著高于对照组(94.4%vs75.O%。P〈0.05).死亡率显著少于对照组(5.6%vs25.0%,P〈0.05);观察组起效时间显著早于对照组(P〈0.05),机械通气时间、胆碱酯酶恢复时间、症状缓解时间及住院时间显著少于对照组(P〈0.05)。结论与阿托品相比,盐酸戊乙奎醚治疗重度有机磷毒物中毒起效快、治愈率高,可减少患者治疗时间。

Abstract:Objective To explore the effect of penehyclidine in treatment of severe organophosphorus poisoning. Methods All of 68 cases with severe organophosphorus poisoning were randomly divided into control group and experience group. Both groups received routine treatment including gastric lavage and pralidoxime iodide treatment. 32 patients of control group were given atropine treatment and 36 cases of experience group received penehyclidine treatment. Mortality, cure rate, onset time, mechanical ventilation time, change of cholinesterase, symptom abatement time and hospital stay were compared between two groups. Results Cure rate in experience group was much higher in experience group than that in control group(94.4% vs 75.0%, P〈0.05); Compared with control group, he mortality in experience group greatly decreased (5.6% vs 25.0% ,P 〈 0.05); Onset time, mechanical ventilation time, change of cholinesterase, symptom abatement time and hospital stay in experience group were much shorter than that in control group (P 〈 0.05). Conclusion Comparison with atropine, penehyclidine for treatment of severe organophosphorus poisoning have faster effect, higher cure rate and less therapy time.
