宫腔镜对IVF-ET失败患者宫腔内病变的诊断价值

目的探讨宫腔镜检查术对体外受精-胚胎移植术（IVF—ET）失败患者的应用价值。方法对379例IVF—ET失败患者行宫腔镜检查。结果在379例IVF—ET失败患者中，宫腔异常者为219例（57．8％），其中子宫内膜息肉或内膜呈息肉样增生者105例（27．7％），宫腔镜下子宫内膜活检病理提示内分泌紊乱者35例（9．2％），宫腔粘连者29例（7．7％），子宫内膜局部呈瘢痕样改变和子宫畸形者各为22例（5．8％），黏膜下子宫肌瘤和子宫腺肌症各为3例（0．8％）。以病理诊断为金标准，宫腔镜对子宫内膜息肉诊断的敏感性、特异性、阳性预测值和阴性预测值分别为71．4％、92．9％、74．1％和91．9％。结论官腔镜应作为IVF—ET失败患者的常规检查。

Diagnostic value of hysteroscopy for uterine cavity lesions in IVF-ET failure women

Objective To explore the importance of hysteroscopy in in-vitro fertilization and embryo transfer （IVF-ET） failures. Methods A retrospective analysis was made on the outcomes of hysteroscopy in 379 cases of IVF-ET failures. Results In 379 cases, 219 （57.8%） had abnormal hysteroscopic findings, including 105 （27.7%） with endometrial polyps, 35 （9.2%） with endocrine disturbance, 29 （7.7%） with intrauterine adhesions, 22 （5.8%） with uterine deformation and endometrial scar, and 3 （0.8%） with submucous myoma and adenomyoma. Thinking of pathologic diagnosis as the standard, the sensibility, specificity, positive predictive value and negative predictive value were 71.4%, 92.9%, 74.1% and 91.9%, respectively. Conclusion Hysteroscopy should be routine examination in IVF-ET failure women.
