后牙区骨组织水平种植体与软组织水平种植体早期边缘骨吸收临床研究

摘    要:目的:对比骨水平种植体和软组织水平种植体在后牙区植入后的早期边缘骨吸收,并分析各分组要素对边缘骨吸收的影响。方法后牙区植入种植体105枚,术后即刻、术后3个月负载前分别进行全景片检查,测量种植体边缘骨高度,计算边缘骨吸收量。应用SPSS13.0软件进行统计分析。结果软组织水平种植体在后牙区近中、远中的边缘骨吸收量分别为(0.594依1.104)mm、(0.298依1.032)mm,骨水平种植体在后牙区近中、远中的边缘骨吸收量分别为(0.889依1.500)mm、(0.603依1.068)mm,两种种植体早期边缘骨吸收差异无统计学意义(P〉0.05)。埋入式与非埋入式愈合方式之间,根形种植体与柱形种植体之间,早期边缘骨吸收差异有统计学意义(P〈0.05)。结论在后牙区,软组织水平种植体负载前边缘骨吸收量与骨水平种植体相近,可以作为后牙区种植的常规选择。非埋入式愈合的患者、使用根形种植体的患者易引起较多的早期边缘骨吸收。

Clinical observation on peri-implant marginal bone loss around bone level implants and tissue level im-plants in posterior region

Abstract:To compare the differences in early marginal bone loss after implant installation in the posterior region between bone level implants and tissue level implants, and to analyze the possible influence factors on peri-implant bone resorption. Methods One-hundred and five implants were placed in patients' posterior re-gion. Peri-implant marginal bone loss was measured on oral panoramic radiographs (OPG) taken immediately after implant placement and three months later. SPSS 13.0 was used for statistic analysis. Results Early marginal bone loss around tissue level implants at mesial and distal sites were(0.594±1.104)mm and (0.298±1.032) mm on aver-age, showing no significant differences with bone level implants (mesial:0.889±1.500mm,distal:0.603±1.068mm;P〉0.05). Significant differences were found between implant healing patterns(submerged and non-submerged) and im-plant shape (tapered and cylinder-shaped)(all P〉0.05). Conclusion Tissue level implants could be used in poste-rior region since their limited marginal bone loss as same as bone level implants. It should be cautious to use non-submerged healing pattern and tapered implants in patients with posterior edentulism.
