结核感染T细胞试验对儿童结核病的诊断价值

目的:评价结核感染T细胞斑点试验(T-SPOT.TB)诊断儿童结核病的应用价值。方法:选择2010年11月至2012年2月昆明市第三人民医院住院的患儿为研究对象。对入院未治疗的32例临床诊断病例的结核病患儿,31例非结核病患儿分别进行T-SPOT.TB试验、结核菌素试验(PPD皮试)、血沉(ESR)、结核抗体(TB-DOT/TB-CK)检测;比较其敏感性和特异性。结果:T-SPOT.TB试验诊断儿童结核病的敏感性为90.6%,特异性为96.8%;PPD试验敏感性为83.3%,特异性为71.4%;TB-DOT/TB-CK敏感性为36.7%,特异性为90.3%;ESR敏感性为63.0%,特异性为26.7%。结论:T-SPOT.TB诊断小儿结核病敏感、快速,而且受其它干扰因素的影响较小,对儿童结核病具有较高的诊断价值,可用于儿童结核病的早期快速诊断。

Diagnostic Value of T-SPOT.TB for Tuberculosis in Children

Objective To explore the diagnostic utility of T-SPOT.TB for tuberculosis in children. Methods From Nov 2010 to Feb 2012, total of 63 consecutive cases of children who were enrolled the third people’s hospital of Kunming were research objects. Peripheral venous blood were collected from 32 cases of children suffering from tuberculosis and 31 cases not TB children who were all untreated for T-SPOT.TB,TB- DOT/TB-CK, ESR and PPD skin test. The results were compared with each other by the computer statistics analysis system. Results The sensitivity and specificity of T-SPOT.TB at diagnosed for tuberculosis in children was 90.6%, 96.8%, respectively; the sensitivity and specificity of PPD skin test were 83.3% respectively 71.4 %; the sensitivity, specificity of TB-DOT/TB-CK were 36.7%, respectively 90.3%; the sensitivity, specificity of ESR were 63.0%, respectively 26.7%. Conclusion T-SPOT.TB is sensitive and fast to diagnose childhood tuberculosis and is less affected by factors, it can be applied in the early diagnosis of childhood tuberculosis.
