经伤椎椎弓根螺钉内固定治疗胸腰段爆裂骨折的疗效研究

目的:探讨经伤椎椎弓根螺钉内固定治疗胸腰段爆裂骨折的临床疗效。方法:57例胸腰段脊柱爆裂骨折患者,分别行经伤椎椎弓根螺钉固定和单纯后路椎弓根螺钉固定,比较两组手术时间、术中出血量、术后引流量、伤口愈合率及伤椎Cobb’s角、前缘高度、椎管占位术后矫正情况。结果:两组手术时间、术中出血量、术后引流量、伤口愈合率均无明显差异(P>0.05),两组术后伤椎Cobb’s角、前缘高度、椎管占位均较术前有明显改善(P<0.01),且伤椎固定组在伤椎Cobb’s角、前缘高度、椎管占位矫正情况均优于单纯后路固定组(P<0.05)。结论:经伤椎椎弓根螺钉内固定较单纯后路椎弓根螺钉固定能更好的矫正伤椎畸形,是一种行之有效的治疗方法。

Pedicle Screw Internal Fixation Via the Injured Vertebra for the Treatment of Thoracolumbar Burst Fractures

Objective To study the feasibility and clinical therapeutic effect of Pedicle screw internal fixation via the injured vertebra for the treatment of thoracolumbar burst fractures.Methods Clinical data about 57 thoracolumbar burst fractures patients,respectively by posterior pedicle screw internal fixation via the injured vertebra and posterior pedicle screw fixation.the operation time,intraoperative blood loss,postoperative drainage,wound healing rate,anterior margin height of fractured vertebra,Cobb’ angle,canal compromise were compared between the two groups.Results No difference was found in operation time,intraoperative blood loss,postoperative drainage,wound healing rate(P > 0.05),Compared with previous operation,the anterior margin height of fractured vertebra,Cobb’ angle,and canal compromise were more significantly improved in via the injured vertebra group than in Simple pedicle screw fixation group after operation(P < 0.05).Conclusion Pedicle screw internal fixation via the injured vertebra of thoracolumbar fractures can achieve better outcome in correcting the Deformity of fractured vertebral than Simple Pedicle screw fixation,and is a practical and effective procedure for the fixation of thoracolumbar fractures.
