B族溶血链球菌感染对胎膜早破、妊娠结局的影响

目的探讨B族溶血链球菌感染对胎膜早破和妊娠结局的影响。方法选取2012年11月～2014年1月本院收治的80例胎膜早破孕妇作为观察组,并选取同期正常妊娠的孕妇100例作为对照组,取阴道和肛拭子应用实时定量PCR进行B族溶血链球菌检测,对比两组妊娠结局,并对观察组中链球菌阳性及阴性孕妇的IL-6含量进行对比分析。结果观察组的链球菌阳性率为15.0%（12/80）,显著高于对照组的6.0%（6/100）,差异有统计学意义（P〈0.05）;链球菌检验阳性孕妇羊水中IL-6含量为（175.9±77.9）ng/L,显著高于检验阴性孕妇的（121.3±58.9）ng/L,差异有统计学意义（P〈0.05）;链球菌检验阳性孕妇不良妊娠结局发生率为66.7%（8/12）,显著高于链球菌检验阴性孕妇的26.5%（18/68）,差异有统计学意义（P〈0.05）。结论 B族溶血链球菌感染是导致胎膜早破的重要因素之一,严重影响妊娠结局,应及时检查并加以控制,以降低母婴发病率。

Influnce of group B Streptococcus infection on premature rupture of membranes and pregnancy outcome

Objective To explore influnce of group B Streptococcus infection on premature rupture of membranes and pregnancy outcome. Methods 80 cases of pregnant women with premature rupture of membranes from November 2012 to January 2014 in our hospital were selected as observation group,100 cases of normal pregnant women for the same period were selected as control group,vaginal and anal swabs real-time quantitative PCR were used to detection of group B Streptococcus,pregnancy outcome in two groups was compared,IL-6 content of positive and negative pregnant women in observation group were compared and analyzed. Results Streptococcus positive rate in observation group was15.0%（12/80）,significantly higher than that in control group[6.0%（6/100）],with statistical difference（P〈0.05）.IL-6 content of amniotic fluid in Streptococcus positive pregnant women was（175.9±77.9） ng/L,significantly higher than that in Streptococcus negative pregnant women[（121.3±58.9） ng/L],with statistical difference（P〈0.05）.The incidence rate of adverse pregnancy outcome in Streptococcus positive pregnant women was 66.7%（8/12）,significantly higher than that in Streptococcus negative pregnant women[26.5%（18/68）],with statistical difference（P〈0.05）. Conclusion Group B Streptococcus infection is one of the important factor that lead to premature rupture of membranes,the serious influence the pregnancy outcome,it shall timely check and control,reduce the incidence rate of maternal and infant.
