腹腔热灌注奈达铂化疗在恶性腹腔积液中的临床疗效及安全性观察

摘    要:目的分析奈达铂与顺铂分别应用于恶性腹腔积液患者的抗癌效果及用药安全性,为该类患者临床诊疗积累实践性经验。方法本院2011年7月~2013年8月收治78例恶性腹腔积液患者,利用随机数字表法进行分组,分别设为研究组和对照组,每组39例。两组患者均接受灌注化疗,其中对照组给予顺铂常规腹腔灌注,研究组采取奈达铂腹腔热灌注化疗,记录并比较两组治疗6个月末不良反应发生情况及腹腔积液控制水平。结果研究组腹水消退有效率高于对照组(P〈0.05)。两组不良反应发生率比较差异无统计学意义(P〉0.05)。结论恶性腹腔积液患者实施腹腔热灌注奈达铂化疗相较于常规腹腔灌注方案,更有利于减轻机体毒副反应,且能有效控制积液水平,值得临床推广。

Clinical efficacy and safety of the peritoneal perfusion nedaplatin chemotherapy in malignant ascites

Abstract:Objective To analyze the clinical effect and drug safety of nedaplatin and cisplatin in the patients with malignant peritoneal and to accumulate practical experience for clinical diagnosis and treatment. Methods 78 patients with malignant peritoneal effusion in our hospital from July 2011 to August 2013 were divided into study group and control group by using random number table method,each group of 39 cases.Perfusion chemotherapy was used in two groups.Cisplatin routine intraperitoneal perfusion was applied in control group,while nedaplatin intraperitoneal hyperthermic perfusion chemotherapy was used in study group.The incidence of adverse reaction and intraperitoneal fluid control level after 6 months of treatment in two groups were recorded and compared. Results In study group,ascites extinction efficiency rate was higher than that in control group(P〈0.05).The incidence rate of adverse reaction in two groups was compared,with statistical difference(P〉0.05). Conclusion Compared with conventional abdominal perfusion solution,intraperitoneal hyperthermic perfusion of nedaplatin chemotherapy can not only reduce the toxicity of the body,but also effectively control the fluid level.
