大肠癌发生过程中Survivin蛋白的表达及其与凋亡、增殖的关系

目的　探讨凋亡抑制蛋白Survivin在大肠癌发生过程中的作用及其与凋亡、增殖的关系。方法　应用免疫组化技术检测了 15例息肉、12例管状腺瘤、16例绒毛状或混合型腺瘤和 5 2例大肠癌组织中Survivin蛋白的表达以及凋亡指数、增殖指数的变化。结果　Survivin蛋白表达与大肠癌临床病理参数无明显的相关关系 ,Survivin蛋白、增殖指数在大肠腺瘤—癌演变中表达均逐渐增高 ,凋亡指数从管状腺瘤至腺癌表达逐渐降低 ,统计学处理差异均有意义。Survivin蛋白表达与增殖指数呈正相关 (P <0 0 0 1) ,与凋亡指数呈负相关 (P <0 0 0 1)。结论　Survivin在大肠癌发生早期过程中表达上调 ,在大肠癌变过程中起重要作用 ,有望成为大肠癌新的早期诊断标志物。

Expression of survivin protein and its relationship with apoptosis and proliferation during human colorectal carcinogenesis

Objective To identify the role of survivin,a novel member of inhibitor of apoptosis protein(IAP)in colorectal carcinogenesis and to determine the correlation of survivin correlated with tumor cell apoptosis and proliferation.Methods Expressions of survivin protein,proliferating cell nuclear antigen labelling index(PCNALI)and apoptoic index(AI) were detected immunohistochemically in humans including 15 polyps,12 tubular adenomas,16 villous adenoma or tubulovillous adenomas,and 52 colorectal carcinomas.Results There was no relationship between survivin protein expression in colorectal carcinoma and sex,tumor size,histological types,lymph-node metastasis,distant metastasis and Duke stage.The immunoreactivity of survivin increased in the transition from polyp to carcinoma( P< 0.001).Similar change was observed for PCNA LI( P< 0.001),but AI decreased from tubular adenoma to carcinoma( P< 0.001).The expression of survivin protein was positively correlated with PCNA LI( P< 0.001),but inversely correlated with AI( P< 0.001).Conclusions These results show that survivin plays an important role in the early development of colorectal cancer and it may become a potential early diagnostic marker.
