开心胶囊对犬心脏停搏复苏后心脏功能的影响

摘    要:目的观察开心胶囊对犬心脏停搏复苏后心脏功能的影响.方法将15只健康杂种犬,随机分为假手术组、模型组和开心胶囊组,复制心脏骤停复苏模型.选择开胸前、除颤后即刻、自主循环恢复5,15,30,60,90,120,180min9个时间点记录各心功能指标.结果模型组与开心胶囊组主动脉平均压(MABP)、左心室作功指数(LVWI)、心肌耗氧量(D-P)、心率(HR)在复苏后的5min一过性增高,15min后MABP、LVWI、心输出量(CO)、心脏指数(CI)和冠脉灌注压(CPP)持续下降,30min后D-P、HR开始下降,与模型组相比,开心胶囊组下降较慢(P<0.05).30min后开心胶囊组CO、CI和CPP与模型组相比仍维持在较高水平(P<0.05).模型组和开心胶囊组MPAP、肺毛细血管楔压(PCWP)、总外周循环阻力(TSVR)与总肺循环阻力(TPVR)在复苏后5min明显上升(P<0.05),而开心胶囊组各指标上升较缓慢,在30min后与模型组相比有显著性差异(P<0.05).复苏后模型组血液温度(T)下降较假手术组、开心胶囊组明显(P<0.05).结论心脏骤停复苏后存在左室收缩功能障碍,导致MABP降低、PCWP增高等血流动力学指标的紊乱.开心胶囊在一定程度上能升高血压和增加心输出量,改善复苏后血液动力学,对心脏骤停复苏后心脏功能具有一定的恢复作用.

Abstract:Objective To study the effect of Kaixin Capsule(KXC) on cardiac function of dogs after cardiac arrest and resuscitation. Methods Fifteen healthy hybrid dogs were randomized into three groups: pseudo- operation group, model group and KXC group.Dog models of cardiac arrest and resuscitation were established. Indexes of cardiac function were examined before opening pectoral cavity, at the very time of post- defibrillation, in 5 th , 15 th , 30 th , 60 th , 90 th , 120 th and 180th min of recovering circulation. Results Mean arterial blood pressure (MABP),left ventricular work index (LVWI), oxygen consumption of myocardium (D- P) and heart rate (HR) in the model group and KXC group temporarily rose 5 min after resuscitation; however, MABP,LVWI,cardiac output (CO),cardiac index (CI) and coronary perfusion pressure (CPP) continued to decrease in 15min; D- P and HR began to decrease in 30 min.As compared with the model group, indexes decreased slowly in KXC group (P < 0.05). In 30 min, the levels of CO,CI and CPP in KXC group still kept higher than those in the model group (P < 0.05). MPAP, pulmonary capillary wedge pressure (PCWP),total systemic vascular resistance (TSVR) and total pulmonary vascular resistance (TPVR) in the model group and KXC group obviously rose 5 min after resuscitation(P < 0.05); however, the above indexes in KXC group went up slowly, which differed from the model group in 30 min (P < 0.05). As compared with KXC group and pseudo- operation group, the blood temperature in model group significantly reduced (P < 0.05). Conclusion Left ventricular contraction dysfunction occurs in left ventricles after cardiac arrest and resuscitation and this lead to the disorder of indexes of hemodynamics such as the decrease of MABP and the increase of PCWP. KXC can increase blood pressure and cardiac output, improve the indexes of hemodynamics after resuscitation and promote the recovery of myocardial dysfunction after cardiac arrest and resuscitation.
