ERCP联合腹腔镜与传统外科手术治疗胆总管结石并胆囊结石的对比研究

目的:探讨分别用ERCP联合腹腔镜胆囊切除术(ERCP+LC)与传统外科手术(胆囊切除+胆总管切开取石+T管引流术)治疗胆总管结石并胆囊结石的治疗效果。方法:ERCP联合腹腔镜组患者35例,传统胆囊切除+胆总管切开取石+T管引流术组31例,对两种治疗方法的住院时间、住院费用、并发症、结石大小、结石数目进行比较。结果:ERCP联合腹腔镜胆囊切除术(ERCP+LC)与传统胆囊切除+胆总管切开取石治疗胆总管结石并胆囊结石均是安全有效的,对胆总管结石大小的选择方面有统计学意义外(P<0.05),其余4项均无统计学意义(P>0.05)。结论:ERCP联合腹腔镜与传统胆囊切除+胆总管切开取石治疗胆总管并胆囊结石均是可取的,但如胆总管结石过大我们可直接考虑第2种治疗方案。ERCP联合腹腔镜是安全有效的,符合外科微创诊治理念,是微创技术的可靠选择。

ERCP Combined Laparoscopic and Conventional Surgical Operation Treatment of Cholecystolithiasis Complicated Comparative Study

Objective:To explore respectively by ERCP combined with laparoscopic cholecystectomy(ERCP+LC)and traditional surgical operation(cholecystectomy plus choledocholithotomy + T tube drainage)treatment of cholecystolithiasis complicated therapeutic effect.Methods:ERCP combined with laparoscopy in 35 patients,the traditional cholecystectomy plus choledocholithotomy + T tube drainage group 31 cases,on two treatment methods of hospitalization time,hospitalization costs,complications,stone size,stone number comparison. The results of ERCP combined with laparoscopic cholecystectomy(ERCP+LC)and the traditional cholecystectomy plus choledocholithotomy in treatment of common bile duct stones and gall stones are safe and effective,for common bile duct stones size selection had statistical significance(P < 0.05),and the remaining 4 had no statistical significance(P > 0.05).Conclusion:ERCP combined laparoscopic cholecystectomy and conventional cholecystectomy plus choledocholithotomy in treatment of common bile duct and gallbladder stones are desirable,but in case of common bile duct stones too big we can consider second kinds of treatment. ERCP combined with laparoscopy is safe and effective,with minimally invasive surgical treatment concept,is a minimally invasive technique with reliable choice.
