2型糖尿病肝损害相关因素研究

目的本研究旨在探讨2型糖尿病肝损害的相关因素。方法回顾性分析该院239例确诊为2型糖尿病患者的临床资料,对其一般资料、肝功能损害情况进行统计分析。结果本组患者共239例,肝功能异常46例,发生率19.2%,其中ALT异常11例(23.9%),而AST以9例(19.6%),ALT及AST均异常29例(63%),AKP异常19例(41.3%),TB、TG、TC均异常升高20例(43.4%),单项TB、TG、TC升高11例(23.9%)。超声、CT或MRI诊断脂肪肝患者23例(50%)。肝功能损害组空腹血糖及血脂均明显高于无肝功能损害组,两组间经过t检验分析差异均有统计学意义,P<0.05。肝功能损害组糖尿病肾病、神经病变、视网膜病变发病率明显高于无肝功能损害组,组间有统计学差异,P<0.05;而酮症酸中毒组间比较无统计学差异,P>0.05。结论高血糖、高血脂是2型糖尿病患者肝损伤最为常见的因素,而糖尿病肾病、神经病变、视网膜病变、酮症酸中毒也易导致肝功能损害,严格控制血糖,降低血脂能极大改善2型糖尿病患者肝损害。

Related factors of Type 2 diabetes liver injury

Objective This study aimed to investigate the relate factors of hepatic damaged caused by type 2 diabetic.Methods Retrospective analysis the clinical data of 239 patients diagnosed type 2 diabetes patients,statistical analysis the general information and impaired liver function.Results In this group of patients with a total of 239 cases,46 cases of abnormal liver function,the occurrence rate of 19.2%,in which ALT was abnormal in 11 cases（23.9%）,and AST in 9 cases（19.6%）,ALT and AST were abnormal in 29 cases（63%）,AKP was abnormal in 19 cases（41.3%）,TB,TG,TC were abnormal elevation in 20 cases（43.4%）,a single TB,TG,TC increased in 11 cases（23.9%）.Ultrasound,CT or MRI diagnosis in 23 patients with fatty liver（50%）.Liver function damage group fasting blood glucose and blood lipid were significantly higher than those without liver dysfunction group,between the two groups after t test analysis had significant differences,P0.05.Impaired liver function group of diabetic nephropathy,neuropathy,retinopathy and ketoacidosis complication rates were significantly higher than those without liver dysfunction group,between the two groups,the complication rate through the chi-square test,including diabetic nephropathy,neuropathy,retinopathy group there were significant differences in P0.05;and ketoacidosis,but there were no statistical differences,P 0.05.Conclusion High blood sugar,high blood lipids are the most common factors in type 2 diabetic patients with liver injury and diabetic nephropathy,neuropathy,retinopathy,ketoacidosis also easily lead to liver damage,strict control of blood glucose,lipid lowering can greatly improve type 2 diabetes mellitus patients with liver damage.
