硫酸吗啡栓纳肛在肛周疾病局麻术中的效果与安全性研究、

为探讨硫酸吗啡栓纳肛在肛周疾病局麻术中的效果与安全性,将2010年6月至2011年6月在局麻下行舡周疾病手术的120例患者随机分为试验组和对照组,各60例。试验组患者术前30min用硫酸吗啡栓20mg纳肛;对照组术前不予特殊处理。两组术中应用2％利多卡因注射液局部浸润麻醉,完成肛周手术。术中根据疼痛程度决定是否追加利多卡因用量,但其总用量均不超过20ml。结果显示,与对照组相比,试验组利多卡因用量少,患者头晕、意识障碍等并发症发生率低,术中疼痛轻,肌松效果好,P〈0．05。结果表明,术前用硫酸吗啡栓20mg纳肛可显著提高肛周疾病术中麻醉效果,且并发症少,适合在门诊肛周疾病手术前推广应用。

Efficacy and Safety of Morphine Sulfate Suppository Used into Anus in Local Anesthesia Operation for Perianal Disease

In order to explore the efficacy and safety of morphine sulfate suppository used into anus in local anesthesia operation for perianal disease,authors divided the 120 patients （from June 2010 yr. to June 2011 yr. ）to receive the operation for their perianal diseases randomly into trial group（ n =60） and control group （n =60）;30min before operation 20mg morphine sulfate suppository was used into anus of the patients of trial group,while control group did not receive special management; in both groups during operation 10ml of 2 % lidocaine injection was used for local infiltration anesthesia, so that to perform the operation for perianal disease ;in the operation whether adding or not lidocaine according to the pain extent of patient, but its dosage was not beyond 20ml. As results,compared with control group in trial group its advantages were less lidocaine dosage,low incidence of such complications as dizziness, disorder of consciousness, etc, mild in traoperative pain, and better muscle relation efficacy （ P〈0.05）. Results show that before operation taking 20mg morphine sulfate suppository used into anus can significantly enhance the anesthesia efficacy in the operation for perianal diseases,and has less complication,thus it is suitable to be popularize clinically before the operation for perianal diseases.
