Bcl-2蛋白在结直肠锯齿状病变伴癌变组织中的表达及其临床意义

为探讨Bcl-2蛋白在结直肠锯齿状病变伴癌变组织中的表达及其临床意义，本研究采用免疫组织化学Envision法检测Bcl-2蛋白在75例锯齿状息肉／腺瘤伴癌变组织及10份健康结直肠黏膜组织中的表达情况。结果显示，Bcl-2蛋白在75例锯齿状息肉／腺瘤伴癌变组织中均有表达，阳性细胞定位于细胞质、细胞膜及细胞核；10份正常结直肠黏膜组织中仅在腺窝基底部有少量细胞见Bcl-2阳性表达，其余腺上皮均不表达。在病变的不同区域，低级别上皮内瘤变、高级别上皮内瘤变及癌变区域中，Bcl-2的阳性表达率分别为44．0％、82．7＇％和78．7％，Bcl-2在癌变及高级别上皮内瘤变区域中的阳性表达率明显高于其在低级别上皮内瘤变区域中的表达（P〈0．05），而Bcl-2在癌变及高级别上皮内瘤变区域中的阳性表达率差异无统计学意义（P〉0．05）。结果表明，Bcl一2蛋白在结直肠锯齿状息肉／腺瘤伴癌变组织中的不同病变阶段有不同程度的表达，Bcl一2蛋白可作为早期诊断结直肠锯齿状息肉／腺瘤伴癌变的重要指标之一。

Expression of Bcl-2 Protein in Colorectal Tissue with Serrated Lesion Associated with Canceration,and its Clinical Significance

In order to explore the expression of Bcl-2 protein in colorectal tissue with serrated lesion associ- ated with canceration, and its clinical significance by using immunohistochemistry Envision method, this study detected the expression of Bcl-2 protein in serrated polyps/adenomas associated with canceration tis sue of 75 patients,and the expression in colorectal mucosa tissue of 10 normal samples. As results,there all were the expression of Bcl-2 protein in the specimen of 75 patients,the positive cells were located at cyto- plasm,cytomembrane and nucleus,while in the specimen of 10 normals there only were less positive cell （i. e,positive expression cell） in the ademo-pouch basement, but no expression in ademo-epithelium;the ex- pression rates in different areas of lesion, low-degree intra-epithelium neoplasm lesion, high-degree intra-ep- ithelium neoplasm lesion, and eanceration areas was 44.0~/~0,82.7o//oo and 78. 7 % respectively, it was seen that the expression in eanceration areas and high-degree intra-epithelium neoplasm lesion was distinctly higher than that in low-degree intra-epithelium neoplasm lesion（ P 〈0.05） ; but there was no significant difference between canceration areas and high-degree intra-epithelium neoplasm lesion（ P 〈0.05）. Results show that there are different extents expressions of Bcl-2 protein during different lesion periods of eolorectal tissue with serrated lesion associated canceration, thus Bcl-2 protein can be served as an important index for early diagnosing colorectal serrated polyps/adenomas.
