进展性缺血性卒中的高危因素观察

目的探讨进展性缺血性卒中的高危因素,为卒中防治提供参考。方法回顾性分析2006年10～2007年12月,我院收治的76例进展性缺血性卒中患者,并以同期收治的75例非进展性缺血性卒中患者作为对照,通过两组患者的年龄、性别、糖尿病史、收缩期血压、血脂、血糖、发热以及白细胞增多和CT早期征象对比。结果进展性卒中患者糖尿病史（P〈0.05）,高血糖（P〈0.001）,收缩压（SBP）过高过低（P〈0.01）,发热和白细胞增多（P〈0.01）,CT早期征象（P〈0.01）,水平明显高于对照组,多变量logistic回归分析表明,高血糖、SBP过高过低、发热、白细胞增多、CT早期梗死征象是卒中进展的高危因素。结论高血糖、血压显著变化（SBP过高过低）、发热、白细胞增多、CT早期梗死征象是进展性卒中的高危因素。

Observation of risk factors for progressive ischemic stroke

Objective To explore the risky factors of proressive ischemic stroke and to provide opinions for its prevention.MethodsFrom october 2006 to december 2007,76 patients with progressive ischemic stroke were analyzed retrospectively and 75 patients with non-progressive ischemic stroke were enrolled as the control group.By comparing their ages,sexes,diabetes history,the levels of systolic blood pressure,lipid,glucose,fever and leukocytosis,signs of early cerebral infarction on CT.ResultsThe proportions of diabetes history（P0.05）,Hyperglycemia（P0.001）,SBP（hyperpiesia/hypopiesia）（P0.01）,fever and leukocytosis（P0.01）,signs of early cerebral infarction on CT（P0.01） were significantly higher in the progressive group.The multivariable logistic regression analysis showed the risky factors of early exacerbation of ischemic stroke were hyperglycemia、 SBP（hyperpiesia/hypopiesia）,fever,leukocytosis and signs of early cerebral infarction on CT.Conclusion The most important risky factors of progressive ischemic stroke are the hyperglycemia、 SBP（hyperpiesia/hypopiesia）,fever,leukocytosis and signs of early cerebralinfarction on CT.
