超声引导内镜在肝外胆管疾病诊治中的临床研究

目的 探讨使用超声替代X-线监视内镜下肝外胆管疾病诊断和治疗的应用价值。方法 超声引导内镜进行鼻胆引流管安置术、ERCP术、肝外胆管冲洗治疗术，并进行X-线、ERCP术对照。结果 (1)超声能清晰监测ERCP管进入肝外胆管内的安放过程；(2)超声ERCP术可提高超声诊断率；(3)超声能全程监测肝外胆管泥沙样结石、肝吸虫团块及炎性沉积物的治疗过程。结论 超声替代X-线监视内镜下肝外胆管疾病诊断和治疗的效果满意，具有极为广阔的发展前景。

Clinical Study on Ultrasound Guided Endoscopic Diagnosis and Treatment of Extra-hepatic Bile Duct Diseases

Objective To expiore the value of ultrasound to replace the X-ray in diagnosis and treatment of the extrahepatic bile duct diseases.Methods We used ultrasound guided endoscopic insertion of the endoscopic retrograde cholangiopancreatographic(ERCP)tube to the extrahepatic bile duct to perform the operation of nasal bile drainage tube setting,the operation of ultrasonic ERCP and the operation of extrahepatic bile duct irrigation and then we compared those with the operation of X-ray ERCP.Results 1.Ultrasound guided endoscope could inspect all the processes of nasal bile drainage tube setting.2.Ultrasound ERCP could improve the diagnosis rate of ultrasound.3.Ultrasound could visualize the courses of the sand-like stone in the extrahepatic bile ducts,liver flukes and the inflammatory exudates.The coincident rate was 100%.Conclusions Ultrasound in the diagnosis and treatment for the extrahepatic bile duct diseases to replace that of the X-ray has boundless and advanced wide prospect.
