摘    要:目的探讨指按天突穴引咳法预防老年开胸术后肺不张的效果。方法对照组33例老年开胸患者采用鼻导管刺激咳嗽排痰,实验组35例老年开胸患者采用指按天突穴引咳法,比较两组患者血气指标改善差值、生理不适和心理反应、排痰效果、肺不张发生率。结果实验组排痰前后血气指标改善差值高于对照组,排痰效果优于对照组,患者的生理不适、心理反应和肺不张发生率均低于对照组(P〈0.01或P〈0.05)。结论采用指按天突穴引咳法可促进老年开胸术后患者痰液的排出,提高血氧分压,有效预防老年开胸术后肺不张的发生,且不会引起生理不适,患者乐于接受。

Effects of cough induced pressing on Tiantu point to prevent senile atelectasis after thoracotomy

Abstract:Objective To explore the effects of pressing on Tiantu point inducing cough to prevent senile atelectasis after thoracotomy. Methods A total of 68 elderly thoracotomy patients were randomly divided into two groups. The control group (n = 33 ) was used a nasal cannula to stimulate cough, and the experimental group (n = 35 ) was adopted the method of pressing on Tiantu point inducing cough. The improved difference of blood oxygen pressure, physical discomfort and psy- chological reactions, expectoration effect and atelectasis rate were compared between two groups after different intervention. Results The improved difference of blood oxygen pressure and the expectoration effects in the experimental group was better than that of the control group. The physical discomfort, psychological reac- tions, and atelectasis rate in the experimental group were lower than that of the control group ( P 〈 0.01 or P 〈 0.05 ). Conclusion Application of pressing on Tiantu point inducing cough in elderly thoracotomy patients can promote sputmn discharge,improve oxygen partial pressure, and prevent senile pulmonary atelectasis after thoracotomy effectively. And it will not cause physical discomfort, so patients are willing to accept.
