老年2型糖尿病合并骨质疏松症患者发病韵相关因素分析

摘    要:目的探讨老年2型糖尿病合并骨质疏松症患者发病的相关危险因素。方法对本院106例老年2型糖尿病合并骨质疏松症患者的年龄、体重指数(BMI)、糖尿病病程与其骨密度(BMD)进行相关因素分析。结果老年2型糖尿病合并骨质疏松症患者中,80岁组的腰椎[(0.756±0.099)g/cm^2]、股骨颈[(0.658±0.111)g/cm^2]及Wards三角[(0.525±0.064)g/cm^2]BMD值较60岁组[(0.883±0.112)g/cm^2、(0.781±0.130)g/cm^2、(0.6274±0.118)g/cm^2]明显下降(P〈0.05)。低体重组(BMI≤20kg/m^2)的腰椎[(0.738±O.114)g/cm^2)]、股骨颈[(0.664±O.112)g/cm^2)]BMD值较正常体重组[(0.816±0.138)g/cm^2、(0.727±0.134)g/cm^2]明显下降(P〈0.05)。糖尿病病程〉10年组的腰椎[(0.743±O.122)g/cm^2]、股骨颈[(0.719±0.147)g/cm^2]BMD值较病程〈5年组[(0.886±0.132)g/cm^2、(0.792±0.122)g/cm^2]明显下降(P〈0.05)。骨密度与年龄、糖尿病病程呈负相关(P〈0.05),与体重指数呈正相关(P〈0.05)。结论老年2型糖尿病合并骨质疏松症患者发病与高龄、低体重指数、糖尿病病程长的危险因素密切相关。

Abstract:Objective To investigate the correlation factors in older type 2 diabetes patients with osteoporosis. Methods The related risk factors including their age, body mass index( BMI), history of dis- ease and bone mineral density( BMD)were analyzed in 106 older type 2 diabetes patients with osteoporosis, who were hospitalized in the first affiliated hospital of Guangzhou Medical College during January 2000 to December 2010. Results BMD of lumbar[ (0. 756 ± 0. 099)g/cm^2 ], neck of femur [ (0. 658 ± 0. 111 ) g/cm^2] and wards triangle[ (0. 525 ± 0. 064)g/cm^2] in the eighty group were distinctly reduced than the sixty group [ (0. 883 ± 0. 112) g/cm^2, (0. 781 ± 0. 130) g/cm2, ( 0. 627 ± 0. 118 ) g/cm^2] ( P 〈 0. 05 ). BMD of lumbar[ (0. 738 ± 0. 114) g/cm^2 ) ] and neck of femur [ ( 0. 664 ± 0. 112 ) g/cm2 ) ] in low weight patients (BMI≤20 kg/m2) were distinctly reduced than normal BMI group [ (0. 816±0. 138 )g/cm^2, (0. 727± 0. 134) g/cm^2 ] ( P 〈 0. 05 ). BMD of lumbar [ ( 0. 743 ± 0. 122 ) g/cm^2 ] and neck of femur [( 0. 719±0. 147) g/cm2 ] in patients with course of diabetes more than 10 years were distinctly reduced than the pa- tients with less course[ (0. 886±0. 132 ) g/cm^2, ( 0. 792 ± 0. 122 ) g/cm^2 ] ( P 〈 0. 05 ). BMD was nega- tively correlated with age and history of diabetes( P 〈 0. 05). BMD was positively correlated with BMI( P 〈 0. 05 ). Conclusions Older, low BMI and long history of diabetes are the risk factors of older type 2 di- abetes patients with osteoporosis.
