胃癌术后腹腔内温热灌洗化疗的临床观察

目的 :探讨胃癌术后腹腔温热灌洗化疗对胃癌术后腹膜转移和生存率的影响。方法 :将胃癌患者随机分为两组各 2 5例。对照组单纯手术 ,研究组手术加术后腹腔温热灌洗化疗 ,方案 :5 Fu +羟基喜树碱 (HPT)观察其并发症及 1～ 3年复发率和生存率。结果 :研究组腹腔 1～ 3年复发率明显低于对照组 ,8%vs 2 0 %,8%vs 32 %及 16 %vs 6 0 %(P <0 0 5 )。 2年及3年生存率明显高于对照组 6 4%vs 48%和 48%vs 32 %(P <0 0 5 )。 1年生存率两组无显著性差异。结论 :胃癌术后腹腔温热灌洗化疗 ,对于防治肿瘤腹膜转移 ,提高患者生存率有重要意义。

Clinical Effect of postoperative Intraperitoneal Hyperthermic Perfusion Chemotherapy

Objective:To analyze the effect of intraperitoneal hyperthermic perfusion chemotherapy on the postoperative metastatic rate and survival rate.Methods:Patients suffered from gastric cancer were randomized into two groups with 25 cases each. Control group, only being treated by operation, study group received intraperitoneal hyperthermic perfusion chemotherapy with the drug of 5-fluorouracil plus hydroxycamptothecin 7-10 days after operation. Side effect and metastasis rate and survival rate were recorded.Results:The peritoneal recurrence rate in 1-3 years were much lower in study group than those of control group, 8% vs 20%, 8% vs 32% and 16% vs 60% (P<0.05), respectively. While 2 and 3 years survival rate were higher, 64% vs 48% and 48% vs 32%(P<0.05), compared with control group. There was no significant difference in 1 year survival rate between two groups.Conclusion:Postoperative Intraperitoneal Hyperthermic Perfusion Chemotherapy may prevent peritoneal metastasis and improve the survival rate.
