翼点入路经侧裂池显微外科手术治疗岛叶胶质瘤

目的 :总结采用翼点入路经侧裂池显微外科手术治疗岛叶胶质瘤的经验。方法 :采用翼点入路和显微外科技术打开侧裂池 ,切除岛叶部位胶质瘤。结果 :本组 10例患者中 ,肿瘤全切除 7例 ,次全切除 3例 ;无继发性神经损害或症状加重 ,无手术死亡 ;术后随访 1个月～ 4年 ,症状继续改善或完全消失 5例 ,无变化 2例 ,加重 1例。结论 :应用显微外科技术经侧裂池入路可避免额颞叶皮质损伤 ,肿瘤切除彻底 ,脑组织损伤小 ,是岛叶肿瘤手术处理中的一种理想入路

Transpterional via sylvian cistern approach microsurgery of gliomas in insula lobe

OBJECTIVE:To summarize the microsurgical experiences and operative technique in the treatment of gliomas in insula lobe transpterional via sylvian cistern approach.METHODS:Exposure and dissection of sylvian cistern transpterional microneurosurgery treatment of gliomas in insula lobe were performed in 10 cases and the data were analysed.RESULTS:Under the operative microscope,the tumors were totally resected in 7 cases,subtotally in 3 cases.No secondary nerve injury and neurological complications were found,and no patient died due to the operation.Nine patients were followed-up from 1 month to 4 years,and remarkable improvement was achieved in 5 cases,no change was found in 2,deterioration in 1 and death in 1.CONCLUSIONS:With the aid of transpterional via sylvian cistern approach combined with the microsurgery technique,brain tissue retraction reduces significantly and the total removal rate for tumors increases.It is a minimally invasive and effective surgical approach for treating gliomas in insula lobe.
