伊曲康唑治疗老年肺部真菌感染的疗效与安全性分析

目的 评价伊曲康唑注射液治疗老年肺部真菌感染的疗效与安全性.方法 采用单中心、开放、前瞻性研究方式,选择老年肺部真菌感染患者应用伊曲康唑静脉注射液治疗,观察用药第7天及用药结束后的临床有效率、真菌清除率及安全性.结果 18例患者纳入该研究,临床诊断12例,占66.7％,确诊6例,占33.3％；假丝酵母菌属感染阳性率为83.3％,曲霉菌属感染阳性率为16.7％,无隐球酵母菌属及其他真菌感染；肺部真菌感染的危险因素中,最常见的为广谱抗菌药物使用,其次为侵入性操作、有创机械通气；用药7d后临床有效率为55.6％,真菌清除率为61.1％,治疗结束时临床有效率为77.8％,真菌清除率为83.3％,显示出良好的治疗效果,不良反应较少.结论 伊曲康唑注射液治疗老年肺部真菌感染具有较好的临床疗效及安全性.

Study on efficacy and safety of itraconazole in treatment of pulmonary fungal infections in elderly patients

OBJECTIVE To observe the clinical efficacy and safety of itraconazole injection in the treatment of elderly patients suffering from acute pulmonary fungal infection. METHODS The research was designed as,open,perspective and experimental trial in single centre.Elderly patients with acute pulmonary fungal infection were selected to receive intravenous intraconazole injection.The clinical efficacy and safety were observed for 7 days. RESULTS Eighteen patients were enrolled in the study,including 6 patients with definite diagnosis and 12 patients being probable.The positive rates of Candida and Aspergillus were 83.3% and 16.7%,respectively,with no cryptococcal and other fungal infections.And the most common risk factor of pulmonary fungal infection is the usage of broad-spectrum antibiotics(83.3%),followed by invasive procedures(catheter/indwelling stomach tube/tracheostomy,(61.1%),invasive mechanical ventilation(50.0%).Clinical analysis showed that the clinical effective rate was 55.6%.and fungal clearance rate was 61.1%.,However,after treatment,the clinical effective rate was 77.8% and fungal clearance rate was 83.3%.Itraconazole injection has good efficacy with few adverse effects. CONCLUSION Itraconazole injection in the treatment of pulmonary fungal infections in elderly patients has good clinical efficacy and safety.
