摘    要:目的 了解医院近年来鲍氏不动杆菌的耐药性变迁及分布,为临床治疗及控制医院感染提供依据.方法 回顾性分析2008-2010年临床分离的538株鲍氏不动杆菌的耐药率及分布.结果 538株鲍氏不动杆菌主要从痰液中检出,占76.2%,鲍氏不动杆菌对抗菌药物的耐药率:头孢哌酮/舒巴坦为29.2%、亚胺培南为41.6%、美罗培南为44.8%、阿米卡星为59.5%、哌拉西林/他唑巴坦为63.0%、头孢吡肟为64.3%、左氧氟沙星为65.6%、头孢他啶为66.4%、环丙沙星为69.5%、庆大霉素为69.9%、磺胺甲噁唑/甲氧苄啶为73.0%、哌拉西林为77.9%、头孢曲松为80.9%,对临床常用的13种抗菌药物的耐药性基本呈逐年增高趋势.结论 应重视动态监测鲍氏不动杆菌的耐药性,根据耐药特点合理选用抗菌药物,预防多药耐药鲍氏不动杆菌在医院内的暴发流行.

Abstract:OBJECTIVE To investigate drug resistance and the distribution of Acinetobacter baumannii in recent years so as to provide basis for clinical treatment and control of nosocomial infections.METHODS A total of 538 stains of A.baumannii were collected from 2008 to 2010,the drug resistance rate and the distribution were analyzed retrospectively.RESULTS Totally 538 strains of A.baumannii were mainly isolated from sputum(76.2%);the drug resistance rates of A.baumannii to cefoperazone/sulbactam,imipenem,meropenem,amikacin,piperacillin/tazobactam,cefepime,levofloxacin,ceftazidime,ciprofloxacin,cidomycin,sulfamethoxazole/trimethoprim,piperacillin,and ceftriaxone were 29.2%,41.6%,44.8%,59.5%,63%,64.3%,65.6%,66.4%,69.5%,69.9%,73.0%,77.9%,and 80.9%,respectively,and the drug resistance of A.baumannii to 13 commonly used antibiotics has been increasing year by year.CONCLUSION We should pay attention to the dynamic monitoring of drug resistance of A.baumannii,and the antibiotics should be chosen according to drug susceptibility testing in the clinical practice so as to prevent the outbreak and prevalence of multidrug-resistant A.baumannii.
