外科手术切口感染的病原菌分布及危险因素分析

目的 探讨外科手术切口感染的病原菌种类及相关危险因素,为预防与控制手术切口感染提供科学依据.方法 对医院发生外科手术切口感染的病例进行病历调查和统计分析.结果 外科手术切口感染与患者的原发基础疾病、年龄、手术季节、手术性质、切口暴露时间、围手术期用药等因素密切相关;共分离出感染病原菌254株,前3位为金黄色葡萄球菌、铜绿假单胞菌、大肠埃希菌,分别占28.4％、24.8％、18.1％,金黄色葡萄球菌中检出耐甲氧西林金黄色葡萄球菌34株,检出率为l 7.2％,耐亚胺培南铜绿假单胞菌检出19株,检出率为30.2％,铜绿假单胞菌中泛耐药株检出8株,检出率为12.7％.结论 必须针对医院感染危险因素采取干预措施,遏制细菌耐药性快速增长的不良趋势,预防与控制手术切口医院感染.

Distribution of pathogenic bacteria causing surgical incision infections and analysis of risk factors

OBJECTIVE To approach the species of pathogenic bacteria and risk factors of incision infections in surgical operation to provide the scientific evidence for prevention and control of it.METHODS The cases of incision infections in surgical operation were investigated and analyzed statistically.RESULTS The incision infections in surgical operation were closely associated with primary diseases,age,seasons,nature of operation,exposure time of incision and perioperative medication.A total of 254 strains of pathogenic bacteria were isolated,of which the top 3 species were Staphylococcus aureus,Pseudomonas aeruginosa and Escherichia coli,accounting for 28.4%,24.8% and 18.1%.Among S.aureus,34 strains of methicillin-resistant S.aureus(MRSA) were isolated,accounting for 47.2%.Nineteen strains of imipenem-resistant P.aeruginosa were isolated,with isolating rate of 30.2%.Eight strains of pan-resistant P.aeruginosa were isolated,with isolating rate of 12.7%.CONCLUSION Intervention should be taken to restrain the rapidly increasing tendency of antimicrobial resistance and prevent and control incision infections in surgical operation.
