系统性红斑狼疮脑脊液抗核抗体测定的临床意义

目的　探讨系统性红斑狼疮 (SLE)患者脑脊液 (CSF)中抗核抗体 (ANA)的定量测定对神经精神性狼疮(NPLE)的诊断价值。方法　对 2 8例SLE患者采用间接免疫荧光法检测CSF及血清中ANA滴度 ,同时进行CSF的常规、生化检测。对NPLE及非NPLE狼疮患者进行统计学分析与比较。结果　两组患者CSF糖、细胞数、氯化物无显著差异 ,NPLE患者CSF的ANA滴度≥ 1:10 0 ,阳性率高于非NPLE组 ;另外NPLE组血中ANA滴度显著高于非NPLE组 ,经治疗后NPLE组CSF中ANA滴度 1:10 0阳性率下降。结论　SLE患者CSF中ANA的定量检测 ,有助于NPLE的早期诊断 ,并为治疗及预后提供参考。

Significance of antinuclear antibody measurement of cerebrospinal fluid in patients with systemic lupus erythematosus

Objective To detect the antinuclear antibody(ANA) titer in cerebrospinal fluid(CSF) in patients with systemic lupus erythematosus(SLE) and evaluate its diagnostic significance to the neuropsychiatric lupus erythematosus(NPLE). Methods ANA was examined with indirect immunofluorescence in the sera and CSF of 28 patients with SLE. The serum levels of glucose and chloride in the CSF were measured simultaneously. Results The serum levels of glucose and chloride in the CSF demonstrated no significant difference between two groups. The positive rate of CSF ANA ≥1:100 in NPLE group was significantly higher than that in non-NPLE. The positive rate of CSF ANA ≥1:100 in NPLE group was reduced after treatment. Conclusion The ANA titer in CSF in patients with SLE is valuable for the early diagnosis of NPLE. It can be used to guide therapy and prognostic determination.
