紫杉醇联合化疗方案治疗晚期胃癌的疗效观察

目的 观察国产紫杉醇(PTX)联合5-氟脲嘧啶(5-Fu)、羟基喜树碱(HCPT)、顺铂(DDP)方案治疗晚期胃癌的疗效和毒副反应.方法 采用PTX 135 mg/m2,加入生理盐水静滴3 h,d1;5-Fu 300 mg/m2,静滴,d1-5;HCPT 8 mg/m2,静滴,d1-5;DDP 20 mg/m2,静滴,d1-5;3～4周为1周期,行2周期治疗后判定疗效.结果 42例中38例可评价疗效,全组完全缓解(CR)1例(2.6%),部分缓解(PR)15例(39.5%),稳定(NC)16例(42.1%),进展(PD)6例(15.8%),有效率(CR PR)42.1%.毒副反应以骨髓抑制、脱发和关节肌肉痛为主,其它毒副反应均较轻微可耐受,无化疗相关死亡.结论 紫杉醇为主方案治疗晚期胃癌疗效肯定,毒副反应能耐受,值得进一步在临床使用.

Clinical Observation of the Treatment of Patients with Advanced Gastric Cancer Mainly by Paclitaxel Combined with Chemotherapy.

Objective To evaluate the response and toxicity of the combination with semimonthly paclitaxel(PTX),5-fluorouracil(5-Fu),hydroxycamptothecin(HCPT) and cisplatin(DDP)in the treatment of patients with advanced gastric cancer.Methods 42 patients with advanced gastric cancer received PTX 135 mg/m2 by intravenous infusion 3 hours,d1;and 5-Fu 300 mg/m2 by intravenous infusion d1-5;or HCPT 8 mg/m2,iv infusion d1-5;or DDP 20 mg/m2,iv infusion d1-5,there were four weeks as one cycle.Efficacy was evaluated at 2 cycles.Results 38 patients were evaluable for efficacy.CR 1(2.6%),PR 15(39.5%),NC 16(42.1%)and PD 6(15.8%).Overall response rate was 42.1%.The main toxicities were myelosupression,alopecia,and other side effects were mild.There were not any death events during the treatment.Conclusion The treatment of advanced gastric cancer mainly by PTX combined with chemotherapy is effective and tolerable toxicity.
