直肠癌全系膜切除结合盆腔自主神经保留术的疗效评价

目的探讨保留盆腔自主神经（PANP）的全直肠系膜切除术（TME）在预防直肠癌术后局部复发中的作用，以及该术式对患者术后排尿和性功能的影响。方法回顾性分析96例直肠癌的临床资料。结果全组无手术死亡，术后随访0．5～5．0年，局部复发7例（7．3％），治疗组患者术后排尿功能障碍和性功能障碍发生率显著低于传统术式组。结论保留盆腔自主神经的全直肠系膜切除术后局部复发率低，能较好保留直肠癌患者术后的排尿和性功能。

Total Mesorectum Excision and Pelvic Autonomic Nerve Plexus Preservation for Rectal Cancer

Objective To investigate the preventive effects of pelvic autonomic nerve preservation（PANP） on local recurrence of rectal eaneer, and the urinary and sexual function in patients undergoing total mesoreetal excision（TME）, Methods Clinieal data of 96 eases of reetal cancer were retrospectively analyzed. Results No operative death was noted. The postoperative follow- up of 0. 5-5.0 years showed that local recurrence oeeurred in 7 ease（ 7.3% ）, the incidences of urinary dysfunetion and sexual dysfunction was much lower than that of traditional operation. Conclusion Total mesoreetal excision with PANP can effectively reduce the local recurrence rate of rectal cancer after operation and the urinary and sexual function is well preserved by PANP.
