围术期并发症对大肠癌肝脏转移患者预后影响的研究

【】目的 探讨围术期并发症对大肠癌肝脏转移患者预后的影响。方法 回顾性分析120例大肠癌肝转移患者的临床资料,根据围术期并发症严重程度将患者分为重度并发症组、轻度并发症组和无并发症组,分析不同等级并发症对大肠癌肝脏转移患者预后的影响。结果120例患者术后有41例出现并发症,根据改良Dindo-Clavien分级标准分为Ⅰ级并发症19例、Ⅱ级并发症6例、Ⅲ级并发症13例、Ⅳ级并发症3例。单因素Log-Rank检验结果提示,原发病灶与肝脏转移间隔时间、肿瘤数目、肝脏转移灶分化程度、肝外转移、有并发症、轻度并发症、重度并发症是影响大肠癌肝脏转移患者预后的影响因素。多因素Cox回归分析分析结果提示,原发病灶与肝脏转移间隔时间、肝外转移、有并发症、重度并发症是影响大肠癌肝脏转移患者预后的独立危险因素。结论 围术期并发症是影响大肠癌肝脏转移患者预后的独立危险因素。

Objective To explore the effect of perioperative complications on the prognosis of patients with colorectal carcinoma liver metastasis. Methods A retrospective analysis was made on the clinical data of 120 cases of patients with hepatic metastasis of colorectal cancer, according to peri operative severity of complications, the patients were divided into severe complication group, mild complication group and non complication group. Analysis of different grades of complications of colorectal cancer with liver metastasis prognosis of patients with. Results 120 cases of patients with postoperative complications occurred in 41 cases, 19 cases of complications of grade I, grade II complications occurred in 6 cases, grade III complications occurred in 13 cases, grade IV complications in 3 cases according to the modified Dindo-Clavien grading standards. The Univariate log rank test showed that, the interval, number of tumor, liver metastasis and the differentiation of the lesion and liver metastasis of primary lesion and liver metastasis, complications, complications were mild and severe complications are influence colorectal cancer liver metastasis factors affecting the prognosis of the patients. Multivariate Cox regression analysis results suggest that the primary lesion and liver metastasis time interval, liver metastasis, complications, severe complication is the effect of colorectal cancer liver metastasis independent risk factors for the prognosis of patients. Conclusions Perioperative complications are the independent risk factors for the prognosis of patients with colorectal cancer liver metastasis..
