脾亢患者血小板计数与脾动脉血流量及血清一氧化氮水平间关系

摘    要:目的 探讨脾亢患者血小板计数与脾动脉血流量及血清一氧化氮(NO)水平间关系.方法 肝硬化伴脾亢患者36例(脾亢组),健康体检者10例(对照组).血清NO浓度测定用硝酸还原酶法,脾动脉血流量由彩色多普勒超声仪测量.结果 脾亢组和对照组血小板计数分别为(60.75±27.77)×10~9/L、(212.40±71.76)×10~9/L(P<0.01);脾动脉血流量分别为(482.50±263.61)、(181.20±61.85)ml/min(P<0.01);血清NO水平分别为(97.646±20.990)、(50.774±19.400)μmol/L(P<0.01).脾亢组血小板计数与脾动脉血流量显著负相关(r=-0.422,P<0.05),血清NO水平与脾动脉血流量呈显著正相关(r=0.746,P<0.01).结论 血清NO水平升高导致脾动脉血流量增加可能在脾亢发生发展中起着重要作用.

Abstract:Objective To investigate the relationship between platelet count with splenic arterial blood flow (SAF) and the plasma nitric oxide (NO) concentration in patients with hypersplenism. Methods Platelet count,SAF and plasma NO concentration were measured in 36 cases of cirrhotic patients with hyper-splenism (hypersplenism group) and 10 cases of healthy volunteers (control group). Results Platelet count in hypersplenism group and control group was (60.75±27.77)×10~9/L and (212.40±71.76)×10~9/L re-spectively (P<0.01) ; SAF was (482.50±263.61) and (181.20±61.85) ml/min (P<0.01) ; plasma NO concentrations were (97.646±20.990) and (50.774±19.400) μmol/L (P<0.01). There was a correla-tion between platelet count and SAF in hypersplenism group (r=-0.422,P<0.05),SAF and plasma NO concentration (r=0.746,P<0.01). Conclusion The increase in SAF induced by the increase of plasma NO concentration may play an important role in the occurrence and development of hypersplenism.
