摘    要:目的检测初发和复发尖锐湿疣(CA)患者皮损及外周血T细胞亚群中TLR9的表达情况,并探讨TLR9在HPV感染和CA复发机制中的作用。方法分别选用30例初发CA和27例复发CA患者皮损,并以28例正常人皮肤石蜡切片作为正常对照组。采用双色免疫荧光抗体染色流式细胞术检测入选者外周血T细胞中TLR9的表达。结果初发CA组和复发CA组皮损颗粒层和棘层TLR9表达(1.38±0.72和2.06±0.84)和外周血CD3+CD4+T细胞内TLR9表达(11.4±3.2)%和(14.6±2.4)%]显著高于正常对照组0.97±0.43和(6.2±2.1)%];另外,复发CA患者外周血CD3+CD4+T细胞内TLR9的表达也显著高于初发CA组;以上差异均有统计学意义(P均<0.05)。但是该两组患者的外周血CD3+CD8+T细胞内TLR9的表达与正常对照组差异不显著(P>0.05)。结论 CA皮损及外周血CD3+CD4+T细胞内TLR9表达上调可能是HPV病毒感染的识别受体,并参与机体的抗HPV免疫应答。TLR9可能在CA复发机制中发挥着重要的作用。

Abstract:Objective To explore the role of TLR9 in HPV infection and condyloma acuminatum (CA) recurrence mechanism through detecting the expression of TLR9 on skin lesions or in peripheral blood T lymphocyte subsets of primary and recurrent CA patients. Methods 30 cases primary CA patients and 27 cases recurrence CA patients with skin lesions were collected, 28 cases of paraffin sections of normal skin were used as normal control. Two-color immunofluorescence antibody staining by flow cytometry technique was used to detect TLR9 expression in normal, primary and recurrent CA patients, peripheral blood T cells. Results In the primary and recurrent CA group, the expression TLR9 on the granular layer and acantholysis of skin lesions (1.38 ± 0.72 and 2.06 ± 0.84) and in CD3+CD4+T cells (11.4±3.2)% and (14.6±2.4)%] were significantly higher than that in normal control group 0.97 ± 0.43 and (6.2 ± 2.1)%]. In addition, the expression of TLR9 in peripheral blood CD3+CD4+T cells of recurrence CA patients was significantly increased compared with the primary CA patients. The above differences were statistically significant ( all P<0.05). However, the differences of the TLR9 expression between in peripheral blood of the patients CD3+CD8+T cells and that in normal control group were not significant (P>0.05). Conclusion The upregulation of TLR9 expression in CA skin lesions and peripheral blood CD3+CD4+T cells may serve as a recognition receptor for HPV infection, and involved in the body’s anti-HPV immune response. TLR9 may play an important role in the CA recurrence mechanism.
