经直肠超声辅助动脉栓塞治疗前列腺增生

目的探讨经直肠彩色多普勒超声对动脉栓塞治疗前列腺增生适应证选择的指导意义。方法比较分析动脉栓塞治疗前列腺增生成功和失败病例的声像图资料，对两组前列腺的大小、突入膀胱的深度、内部血流、供血动脉进行观察比较。结果动脉栓塞成功病例治疗前：前列腺大小96．5～187．6ml，平均127．3ml；突入膀胱的深度0．9．3．5cm，平均2．3cm；内部血流信号2～3级；最少有1支以上明确的供血动脉。失败病例：前列腺大小52．3～117．5ml，平均101．5ml；突入膀胱的深度0．1．8cm，平均0．6cm；内部血流信号1～2级；无明确的供血动脉。结论经直肠彩色多普勒超声，术前可对前列腺的情况做出正确评估，对动脉栓塞治疗前列腺增生，在适应证的选择上具有重要的指导意义。

Treatment of prostate gland hyperplasia with arterial embolism and transrectal ultrasound

Objective To explore the clinical value of transrectal color Doppler ultrasound in prostate gland hyperplasia treat- ed by arterial embolism. Methods The sonograms of successful cases and failed cases treated by arterial embolism were analyzed and compared. The volume of prostate, the depth of prostate intruding the bladder, interior blood flow and feeding artery in two groups were analyzed. Results In success operation group, the average volume of prostate was 127.3ml, the average depth of prostate intruding the bladder was 2.3cm, the signal of inner bloodstream was ranged from grade 2 to grade 3, at least one feeding artery was detected. In failure group, the average volume of prostate was 101.5ml, the average depth of prostate intruding the bladder was 0.6cm, the signal of inner bloodstream was ranged from grade 1 to grade 2, no definite feeding artery was detected. Conclusion Transrectal color Doppler ultrasound can evaluate the situation of prostate correctly, it also has great clinical value in treating prostate gland hyperplasia by arterial embolism.
