老年2型糖尿病个体化营养治疗的意义

摘    要:目的研究个体化营养治疗对于老年2型糖尿病的临床效果。方法选择2008年11月。2010年10月浙江省舟山医院共收治的124例老年2型糖尿病患者为研究对象,采用对比治疗的方法,按照1:1的人数比例分为实验组和对照组,每组各62例。实验组给予个体化营养治疗,对照组采用常规方法进行治疗,观察两组患者治疗后免疫球蛋白A(I啦)、免疫球蛋白G(IgG)、C型反应性蛋白(CRP)、微量白蛋白(ALB)、血红蛋白(Hb)、空腹血糖(FPG)和餐后2h血糖(2hPG)情况,比较两组患者的治疗效果。结果免疫学指标:治疗后两组患者的IgA、IgG、ALB、Hb水平均较治疗前均明显升高,差异有统计学意义(P〈0.05),CRP比治疗前显著降低,差异有统计学意义(P〈0.05);实验组治疗后IgA、IgG、ALB、Hb(3.3±1.4)、(14.5±3.2)、(38.7±1.3)、(130.6±35.5)g/L]明显高于对照组(2.4±1.2)、(13.5±4.5)、(35.5±2.4)、(118.5±21.8)g/L],差异有统计学意义(P〈0.05)。血糖指标:治疗后两组患者FPG、2hPG均比治疗前显著下降(P〈O.05);实验组FPG、2hPG(8.1±1.6)、(11.3±2.4)mmol/L]低于对照组(9.4±5.5)mmol/L、(12.5±7.8)mmol/L],差异有统计学意义(P〈0.05)。结论个体化营养治疗能有效降低老年2型糖尿病患者的血糖,提高其免疫功能,提高患者的生存质量,值得临床推广。

Abstract:Objective To study the clinical efficacy of individualized nutrition therapy for elderly type 2 diabetes mellitus pa- tients. Methods 124 cases of elderly type 2 diabetes mellitus in Zhoushan Hospital of Zhejiang Provinc from November 2008 to October 2010 were selected as study object, contrasting treatment was adopted in these patients, and they were divided into experimental group and control group, with 62 cases in each group according to the proportion of 1:1. The experimental group was given individualized nutrition therapy, the control group was treated by conventional method. IgA, IgG, CRP, ALB, Hb, FPG and 2hPG of two groups were observed after treatment, and the curative effect of two groups were compared. Results Immunological parameters: the IgA, IgG, ALB, Hb of two groups after treatment were significantly higher than those before treatment, the differences were statistically significant (P 〈 0.05), while CRP decreased significantly than that before treatment (P 〈 0.05); IgA, IgG, ALB, Hb (3.3±1.4), (14.5±3.2), (38.7±1.3), (130.6±35.5) g/L] of experimental group were significantly higher than those in control group (2.4±1.2), (13.5±24.5), (35.5±2.4), (118.5±21.8) g/L] after treatment, the differences were statistically significant (P 〈 0.05). Glycolic index: FPG, 2hPG of two groups after treatment decreased significantly than those before treatment (P 〈 0.05), FPG, 2hPG of experimental group after treatment (8.1± 1.6), (11.3±2.4) mmol/L] were lower than those in control group (9.4~5.5), (12.5±7.8) mmol/L], the differences were statistically significant (P 〈 0.05). Conclusion Individualized nutrition therapy can effectively reduce the level of blood glucose of patients with elderly type 2 diabetes mellitus, improve their immune function, and quality of life, is worthy of clinical promotion.
