头位剖宫产中胎头高浮时改良式托取胎头40例临床分析

摘    要:目的探讨头位剖宫产中胎头高浮时托取胎头的新方法。方法将头位胎头高浮剖宫产分成原先的传统常规手法托取胎头38例(对照组),及目前改良的托取胎头手法40例(改良组)。在手术并发症,如子宫切口撕裂、胎头娩出难易程度、术中出血量、新生儿出生时Apgar评分等方面进行对比分析研究。结果头位剖宫产中胎头高浮时应用改良式托取胎头方法具有容易取出胎头、避免子宫切口撕裂、减少手术中出血、提高新生儿出生评分等优点。结论剖宫产胎头高浮时用改良式托取胎头方法能避免不必要的对母婴的医源性手术损伤.值得推广.

Abstract:Objective To investigate a new maneuver to gentle traction of fetal head in cesarean section when the fetal head is in high position. Methods From December 2007 to Apil 2008, 78 cases of ceasarean section fetal head in high position were randomly divided into traditional maneuver group and modify maneuver group. Results Cesarean section was a most often operative delivery in obstetrics. When the fetal head was in high position, it was hard to tract out at operation. It often used forceps to tract out fetal head in ceasarean section. So it had a lot of complications on and postoperation did harm to fetus and pregnancy woman. We had researched a modified maneuver to tract out the fetus. It decreased the complications remarkably from ceasarean section. It tracted fetal head easily, decreasing bleed of uterus and rupture of uterine incision. The maternal and fetus had no injureies from operation. Conclusions Modified maneuver to gentle traction of fetal head in ceasarean section can avoid injureies from operation for maternal and fetus, which deserves to popularize in clinical application.
