脂微球前列腺素E1改变糖尿病周围神经病变患者神经传导速度随机对照试验的Meta分析

摘    要:目的:评价脂微球前列腺素E1治疗糖尿病周围神经病变(DPN)的疗效与安全性。方法:计算机检索OVID-LWW、CNKI和万方数据库,收集以脂微球前列腺素E1为干预措施治疗糖尿病周围神经病变的随机对照试验研究(RCT)。对纳入研究进行方法学质量评价,提取有效数据进行Meta分析。结果:共纳入11个RCT,共978例糖尿病周围神经病变患者。试验组为普通基础治疗加脂微球前列腺素E1,对照组为基础治疗或基础治疗加B族维生素。Meta分析结果显示,试验组在改善DPN患者的神经传导速度方面优于对照组。治疗后,正中神经运动神经传导速度(MCV)和腓总神经MCV的加权均数差(WMD)及其95%CI分别是4.39(2.86,5.91)和3.38(2.21,4.54)。正中神经感觉神经传导速度(SCV)敏感性分析的WMD及其95%CI为3.55(1.86,5.24)。结论:脂微球前列腺素E1对DPN患者的神经传导速度可能有改善作用。因纳入文献的试验方法学质量较低,仍需开展科学规范、高质量、大样本、长期随访的RCT。
