急性白血病中血清CA125的临床意义

目的 研究急性白血病中血清CA125水平,并探讨其临床意义.方法 收集我院血液内科确诊急性白血病94例,以同期收入的慢性再生障碍性贫血患者为对照,分析患者不同阶段血清CA125水平的变化,探讨血清CA125与临床特征、疗效及预后的关系.结果 急性淋巴细胞白血病（ALL）患者血清CA125水平升高占37.5％（12/32）,患者初治时血清CA125水平较完全缓解期及对照组升高,统计学有差异（P＜0.05）,完全缓解期与对照组差异无统计学意义,并且ALL初治时血清CA125水平与胸骨压痛、乳酸脱氢酶（LDH）、β2微球蛋白（β2-MG）、肝脾肿大或淋巴结肿大有正相关性（P＜0.05）,血清CA125水平在急性髓细胞白血病（AML）中与对照组比较其差异无统计学意义.结论 血清CA125水平可一定程度反映ALL的病情活动和治疗反应,可能反映ALL的侵袭活性.

Clinical value of serum CA125 in patients with acute leukemia

Objective To study serum CA125 level of patients with acute leukemia and its clinical value. Methods 94 cases of acute lymphoblastic leukemia （ALL） were collected, the inpatients with chronic aplastie anemia were chosen as control group at the same time, to analyze the level of serum CA125 in different status of ALL and to learn the relationship between clinical features, treatment, prognosis and serum CA125. Results 37.5% （12/32） of newly diagnosed ALL patients haw： elevated average serum CA125 levels, serum CA125 levels were normal in the control group, serum CA125 levels in newly diagnosed ALL patients were significantly higher than complete remission patients and the controls （P 〈 0. 05 ）, There was no signifi cant difference between the serum CA125 levels of complete remission patients and the controls. Correlation studies revealed that serum CA125 level had significant positive correlations with sternal tenderness, serum LDH （Lactate dehydrogenase） or β2-MC level, splenohepatomegalia or lymphadenectasis. There is no statistical difference in serum CA125 level between AML patient, and the controls. Conclusion Serum CA125 level may has some clinical value in monitoring response to treatment and estima. ting prognosis in patients with ALL, and may reflect the invasion of ALL.
