上气道咳嗽综合征中医证素特点的临床研究

目的探讨上气道咳嗽综合征(UACS)患者中医证素特征。方法采用"证素辨证"方法 ,提取患者病位、病性证素,对其进行统计分析,探讨本病的中医病理特征。结果 UACS病位证素肺频数显著高于表、肝、胃;病性证素痰频数显著高于热、气滞、湿、寒、饮、阴虚、阳虚、气虚、血虚;并且只有肺和痰两个证素平均积分超过150,为重度病变。结论 UACS核心病位在肺,可兼表、肝、胃;病性以痰为主,寒热虚实互见,肺气郁而生痰是UACS的核心病理机制。

A Clinical Study on Characteristics of TCM Syndrome Elements in Upper Airway Cough Syndrome

Objective: To investigate the characteristics of syndrome elements in patients with upper airway cough syndrome(UACS).Methods: Patients with UACS who met the inclusion and exclusion criteria of UACS were observed by syndrome elements differentiation methods.The syndrome elements of the disease location and the disease nature were extracted by syndrome elements differentiation.The features of TCM pathology of UACS were analyzed with mathematical statistics methods.Results: The syndrome element of the disease location lung appeared more frequently than exterior,liver and stomach.The syndrome element of the disease nature phlegm appeared more frequently than heat,qi stagnation,dampness,cold,retention of fluid,yin asthenia,yang asthenia,qi asthenia and blood asthenia.And only the average integrals of syndrome elements lung and phlegm were above 150,which were regarded as severe lesion.Conclusion: The core disease location of UACS is lung,which could company with exterior,liver and stomach.The primary disease nature of UACS was phlegm which could be concurrence of cold and heat,deficiency and excess.The production of phlegm due to lung qi depression is the key pathological mechanism of UACS.
