肾结核142例临床分析

目的:探讨肾结核的早期诊断和治疗.方法:回顾性分析142例肾结核患者的临床资料.结果:膀胱刺激征和血尿是最常见的临床症状.尿查抗酸杆菌、PCR-TB-DNA、PPD-IgG检查阳性率分别为39.1%、49.1%和64.2%;IVU、B超、CT的诊断阳性率分别为70%、29.5%和78.5%.接受药物治疗的病例(占47.1%)均获痊愈,随访15～20个月,62例手术治疗均获痊愈,随访1～5年.结论:联合运用多项实验室和影像学检查,可以明显提高诊断肾结核的敏感性,药物治疗肾结核效果满意,肾结核肾切除应尽可能切除同侧输尿管.

Clinical Analysis of 142 Cases of Renal Tuberculosis

Objective: To study the diagnoses and treatment of renal tuberculosis(TB).Method:A retrospective study was made on 142.Result:Irritation symptoms and hemat-uria were the most common symptoms.The positive result of acid-fast stains,urinary PCR-TB-DNA,PPD-IgG were 39.1%,49.1% and 64.5% respectively The diagnostic accuracy of IVU,B-type ultrasonography and CT were 70%,29.5% and 78.5%.All of the case who received medications (47.1%),All of the 62 case who received surgery recovered com-pletely.Conclusion: Combined use of different means of diagnostic technique would help to assess the diagnosis of renal TB espeiall in atypical case,medications yiels satisfactory outcome for renal tuberculsis.When the renal is resected,the involvedureter should be excised as much as possible.
