糖尿病肾病微血管病变的超声与临床研究

摘    要:目的观察糖尿病肾病(DN)肾脏局部血流动力学的变化情况,探讨彩色多普勒血流显像(CDFI)在DN诊疗中的临床应用价值。方法应用CDFI检测35例肾功能不全期DN和年龄匹配的35例临床DN期患者肾脏弓状动脉血流动力学变化的各项参数,并与30例正常对照组对比分析。结果肾功能不全期DN患者搏动指数(PI)和阻力指数(RI)高于临床DN期患者P<0.01和0.05,收缩期峰值流速、平均流速低于临床DN期P<0.05和0.01。与正常组比较,二期DN的PI、RI明显增高P<0.01,肾功能不全期DN舒张末期流速和平均流速明显降低P均<0.01,临床DN期收缩期峰值流速增高P<0.05,舒张末期流速降低P<0.05。结论CDFI是评价DN肾脏局部的血流动力学变化的有效方法,在预测肾脏损害和判断肾功能不全方面有一定价值。

Abstract:Objective To observe the changes of renal hemodynamics in diabetes mellitus with diabetic nephropathy(DN), investigate the clinical value of color Doppler flow imaging(CDFI)in patients with DN. Methods The renal hemodynamics of arcuate artery flow were studied with CDFI in 35 patients with diabetic renal failure age matched 35 cases of clinical DN and 30 cases of normal volunteers. Results The pulsative index (PI)and resistance index (RI) of the patients with diabetic renal failure were significantly higher and the systole peak velocity and mean velocity were lower than those of clinical DN. Compared with normal controls, the PI and RI in the patients with clinical DN or diabetic renal failure were greatly increased. The end-diastolic velocity of blood flow and the mean velocity were significantly reduced in the patients with diabetic renal failure. The peak velocity of blood flow during systole was increased and the end-diastolic velocity of blood flow was reduced in the patients with clinical DN. Condusion CDFI is proved to be an effective method in evaluation of renal hemodynarnics in DN, and CDFI might be valuable in forecasting the renal injury and judging the renal failure.
